摘　　要：目的研究CD138的表达对慢性子宫内膜炎合并不孕症患者的临床诊断价值。方法选择2019年7月至2020年6月在河源市妇幼保健院接受子宫内膜活检的85例不孕症患者为研究对象,所有患者均接受苏木精伊红染色检查,并用免疫组织化学染色检查CD138的表达情况,依据苏木精伊红染色形态学诊断结果将所有患者分为慢性子宫内膜炎阴性组(阴性组)38例、慢性子宫内膜炎疑似组(疑似组)31例和慢性子宫内膜炎阳性组(阳性组)16例,比较三组患者的CD138表达水平,按照CD138表达情况将所有患者分为CD138阴性组(n=60)与阳性组(n=25),并比较两组患者的妊娠情况。结果阳性组患者的CD138阳性率为100.00%,明显高于疑似组的25.81%和阴性组的2.63%,且疑似组的CD138阳性率明显高于阴性组,差异均有统计学意义(P<0.05);CD138阳性组患者的妊娠率为36.00%,明显高于CD138阴性组的11.67%,差异有统计学意义(P<0.05)。结论临床通过免疫组织化学染色检查CD138的表达情况可有效识别子宫内膜中浆细胞,为慢性子宫内膜炎合并不孕症的诊断提供参考依据,从而对不孕症的治疗可能有一定指导价值。Objective To study the clinical diagnostic value of CD138 expression in patients with chronic endometritis complicated with infertility. Methods A total of 85 infertility patients who received endometrial biopsy in Heyuan Maternal and Child Health Hospital from July 2019 to June 2020 were selected as the research objects. All patients were examined by hematoxylin eosin staining, and CD138 expression was examined by immunohistochemical staining examination, according to morphological diagnosis results of hematoxylin eosin staining, all patients were divided into chronic endometritis negative group(n=38), chronic endometritis suspected group(n=31) and chronic endometritis positive group(n=16), the expression level of CD138 in the three groups was compared. All the patients were divided into CD138-negative group(n=60) and CD138-positive group(n=25) according to the expression of CD138, and the pregnancy of the two groups was compared. Results The positive rate of CD138 in the positive group was 100.00%,which was significantly higher than 25.81% in the suspected group and 2.63% in the negative group, and the positive rate of CD138 in the suspected group was significantly higher than that in the negative group;the differences were statistically significant(P<0.05). The pregnancy rate of CD138 positive group was 36.00%, which was significantly higher than 11.67% of CD138 negative group(P<0.05). Conclusion Clinical examination of CD138 expression by immunohistochemical staining can effectively identify plasma cells in endometrium and provide reference for the diagnosis of chronic endometritis complicated with infertility, which may have certain guiding value for the treatment of infertility.
