LMR、PLy A、C1q与AMI急诊PCI患者心肌灌注水平的关系及联合预测术后MACE的ROC分析  被引量:2

摘  要:目的探讨淋巴细胞与单核细胞比值(LMR)、血小板-淋巴细胞聚集体(PLy A)、血清补体成分1q(C1q)与急性心肌梗死(AMI急诊经皮冠脉介入治疗(PCI)患者心肌灌注水平的关系及联合预测术后主要心血管不良事件(MACE)的价值。方法前瞻性选取2017年2月至2020年3月拟行急诊PCI治疗的AMI患者205例,根据PCI后即刻心肌灌注水平分为观察组(n=31,TIMI分级≤Ⅱ级)、对照组(n=174,TIMI分级Ⅲ级),比较两组一般资料、LMR、PLy A、C1q水平。结果观察组LMR低于对照组,PLy A、C1q高于对照组(P<0.05);LMR与TIMI分级呈正相关,PLy A、C1q与TIMI分级呈负相关(P<0.05);LMR、PLy A、C1q预测MACE的曲线下面积(AUC)分别为0.734、0.828、0.800,各指标联合预测MACE的AUC为0.882(P<0.05)。结论检测PCI术前LMR、PLy A、C1q对AMI急诊PCI患者心肌灌注水平评估、MACE的预测具有一定意义。Objective To explore the relationship between the lymphocyte to monocyte ratio(LMR),platelet lymphocyte aggregates(PLy A),complement 1 q(C1 q)and the myocardial perfusion level of emergency percutaneous coronary intervention(PCI)patients with acute myocardial infarction(AMI)and the value of joint prediction of major adverse cardiovascular events(MACE)after surgery.Methods From February 2017 to March 2020,a total of 205 AMI patients planned to undergo emergency PCI treatment in our hospital were prospectively selected.According to the level of myocardial perfusion immediately after PCI,they were divided into observation group(n=31,TIMI grade≤Ⅱ)and control group(n=174,TIMI gradeⅢ).The general data,LMR,PLy A,and C1 q levels of the two groups were compared,and the data were statistically analyzed.Results The LMR of the observation group was lower than that of the control group,and PLy A and C1 q of the observation group were higher than those of the control group(P<0.05).LMR was positively correlated with TIMI grade,and PLy A and C1 q were negatively correlated with TIMI grade(P<0.05).The area under the curve(AUC)of MACE predicted by LMR,PLy A,and C1 q were 0.734,0.828 and 0.800,respectively,and the combined AUC of each index was 0.882(P<0.05).Conclusion The detection of LMR,PLy A,and C1 q before PCI has certain significance for the assessment of myocardial perfusion level and the prediction of MACE after emergency PCI patients with AMI.
