摘 要:消化道恶性肿瘤(gastrointestinal cancer,GIc)的发病率和死亡率位居各类肿瘤前列,给社会带来沉重的医疗负担,而肿瘤相关血栓栓塞是这类患者最严重的心血管并发症之一,以肿瘤相关静脉血栓栓塞(cancer-associated venous thromboembolism,CAVTE)最为常见,主要包括由肿瘤导致的深静脉血栓栓塞与肺栓塞。肿瘤患者CAVTE事件的发生率可达非肿瘤患者的4~6倍,包括胃癌、胰腺癌在内的GIc是CAVTE的高危肿瘤类型。GIc患者CAVTE的发生机制包括以下两类:肿瘤细胞本身产生促凝物质以及肿瘤细胞与正常体细胞相互作用导致的血小板活化及凝血系统的激活。为识别CAVTE高危患者,目前已有数个评分系统可用于预测CAVTE的发生,然而尚无针对GIc患者的CAVTE预测评分。由此,本文将重点对GIc患者CAVTE的发生机制与风险评估方法进行综述。The morbidity and mortality of gastrointestinal cancers(GIc)rank at the top of various types of cancers and bring heavy healthcare burden.Cancer associated thromboembolism is one of the most serious cardiovascular complications in these patients,and cancer associated venous thromboembolism(CAVTE),which includes deep venous thrombosis and pulmonary thromboembolism,is the most common type.Patients with cancer are 4-6 times more likely to develop CAVTE than in general population,and gastric cancer and pancreatic cancer are high-risk cancer types for CAVTE.The mechanisms of CAVTE in GIc patients mainly include two categories:the secretion of procoagulant by cancer cells,and the activation of platelets and coagulation system caused by the interaction between cancer cells and normal cells.To identify the patients with a high risk of CAVTE,several prediction models are currently available,however,there is no prediction model specifically for GIc.Thus,we’d like to review the molecular mechanisms and the risk assessment for CAVTE in patients with GIc.
