CT及MRI显示卵巢血管和输卵管在诊断盆腔肿瘤中的应用分析

摘　　要：目的:探讨CT及MRI显示卵巢血管和输卵管在诊断盆腔肿瘤中的应用,为临床诊断提供一定参考。方法:本次回顾性选择2018年1月~2020年2月甘州区人民医院经CT及MRI检查的盆腔肿瘤患者61例(84个肿瘤),术后病理证实为卵巢源性65个(恶性51个和良性14个)及非卵巢源性19个,分析CT及MRI显示卵巢血管和输卵管对盆腔肿瘤定位及定性的诊断价值,并分析CT及MR与术后病理结果的一致性。结果:61例盆腔肿瘤患者中共84个肿瘤病灶,70个显示141条血管进入肿瘤内部或相连,其中24个显示1条,21个显示2条,25个显示3条。影像学检查可见:卵巢源性肿瘤70个和非卵巢源性肿瘤14个。65个卵巢源性肿瘤中影像学检查提示恶性交界性46个和良性19个。采用CT及MRI检查对盆腔肿瘤的定位准确率84.52%、灵敏度93.85%、特异度52.63%、假阳性率47.37%、假阴性率6.15%,表明一致性高(Kappa=0.801)。采用CT及MRI检查对卵巢源性肿瘤的定性准确率76.92%、灵敏度80.39%、特异度64.29%、假阳性率35.71%、假阴性率19.61%,表明一致性高(Kappa=0.784)。结论:CT及MRI显示卵巢血管和输卵管对盆腔肿瘤定性及定位诊断有较高价值,为早期诊断提供参考。Objective:To explore the application of CT and MRI in the diagnosis of pelvic tumors,To provide some reference for clinical diagnosis.Methods:From January 2018 to February 2020,61 patients(84 tumors)with pelvic tumors underwent CT and MRI examination in Ganzhou District People’s Hospital were selected retrospectively,postoperative pathology confirmed 65 cases of ovarian origin(51 malignant and 14 benign)and 19 cases of non ovarian origin,objective to analyze the value of CT and MRI in the diagnosis of pelvic tumors,the consistency of CT and MRI with postoperative pathological results was analyzed.Results:There were 84 tumor foci in 61 patients with pelvic tumor,of which 70 showed 141 vessels entering or connecting with the tumor,24 showed 1 vessel,21 showed 2 vessels and 25 showed 3 vessels.Imaging examination showed 70 ovarian tumors and 14 non ovarian tumors.Of the 65 ovarian tumors,46 were borderline malignant and 19 benign.The localization accuracy of CT and MRI for pelvic tumors was 84.52%,the sensitivity was 93.85%,the specificity was 52.63%,the false positive rate was 47.37%,and the false negative rate was 6.15%,indicating high consistency(Kappa=0.801).The qualitative accuracy rate of CT and MRI for ovarian tumors was 76.92%,the sensitivity was 80.39%,the specificity was 64.29%,the false positive rate was 35.71%,and the false negative rate was 19.61%,indicating high consistency(Kappa=0.784).Conclusion:CT and MRI display of ovarian vessels and fallopian tubes is of high value in the qualitative and localized diagnosis of pelvic tumors,which provides a reference for early diagnosis.
