摘 要:目的探讨Toll样受体4(TLR4)/核转录因子-κB(NF-κB)信号通路在鼻炎灵治疗过敏性鼻炎(AR)中的作用。方法选取2019年2月—2020年2月我院收治的35例AR(观察组),均采用鼻炎灵治疗;同期另选取我院体检健康者35例作为对照组。收集2组静脉血和鼻腔鼻丘区域黏膜组织,检测2组静脉血炎性因子[干扰素-γ(IFN-γ)和白细胞介素-4(IL-4)]及免疫球蛋白E(IgE)水平,采用免疫组织化学染色法检测观察组鼻腔黏膜组织相关因子表达,采用qRT-PCR和Western blot法检测2组鼻腔黏膜组织TLR4/NF-κB mRNA和蛋白表达水平。结果观察组患者治疗后主要临床症状明显改善,外周血嗜酸粒细胞绝对值及百分比下降(P<0.01)。观察组患者治疗前鼻腔黏膜组织标本均可见较多的IL-4和IgE阳性染色细胞,治疗后开始减少,而IFN-γ阳性细胞数增多。观察组患者治疗后IFN-γ表达水平较治疗前显著升高,IL-4和IgE表达水平较治疗前降低(P<0.05)。观察组患者治疗后TLR4/NF-κB mRNA及蛋白表达显著降低(P<0.01)。结论鼻炎灵可能通过抑制TLR4/NF-κB信号通路活性,下调AR患者鼻腔黏膜组织中炎症相关因子mRNA、蛋白表达,从而产生治疗效果。Objective To investigate the effect of toll-like receptor4(TLR4)/nuclear transcription factor-κB(NF-κB)signal pathway in treatment of patients with allergic rhinitis(AR)by using Biyanling.Methods A total of 35 patients with AR admitted between February 2019 and February 2020 were enrolled as observation group and gave Biyanling.Meanwhile,another 35 healthy persons undergoing physical examinations were selected as control group.The venous blood and mucosal tissues of nasal cavity and nasoturbinal concha were collected,and inflammatory factors[interferon-γ(IFN-γ)and interleukin-4(IL-4)]and immunoglobulin E(IgE)in venous blood were detected in two groups.Expressions of related factors in nasal cavity and nasoturbinal concha were detected by using immunohistochemical staining in observation group,and mRNA and protein expressions of TLR4/NF-κB nasal cavity and nasoturbinal concha were detected by using quantitative real time-polymerase chain reaction(qRT-PCR)and Western blot in two groups.Results After treatment,the mainly clinical symptoms were significantly improved,and the absolute value and percentage of peripheral blood eosinophils were significantly decreased in observation group(P<0.01).In observation group,more IL-4 and IgE positive staining cells could be seen in nasal cavity and nasoturbinal concha before treatment,which began to decrease after treatment,while the number of IFN-γpositive cells was increased.After treatment,expression of IFN-γwas significantly increased,while expressions of IL-4 and IgE were significantly decreased compared with those before treatment in observation group(P<0.05).Expressions of TLR4/NF-κB mRNA and protein after treatment were significantly decreased compared with those before treatment in observation group(P<0.01).Conclusion Biyanling may achieve therapeutic effect by inhibiting activity of TLR4/NF-κB signal pathway,down-regulate expressions of inflammation-related mRNA and protein in nasal cavity and nasoturbinal concha of AR patients.
