摘 要:目的了解从四川省急性弛缓性麻痹(acute flaccid paralysis,AFP)病例和手足口(hand-foot-and-mouth disease,HFMD)病例中分离到的柯萨奇病毒B组3型(Coxsackievirus B3,CVB3)的基因特征,探索其变异规律及在四川省的传播动态。方法对2008—2020年AFP病例和2013—2019年非肠道病毒71型(Enterovirus 71,EV 71)和柯萨奇A16型(Coxsackie virus A16,CVA 16)感染的HFMD病例标本进行细胞分离培养,血清学鉴定,全VP1基因序列的测定及核苷酸同源性和遗传进化的分析,用MEGA6.0软件以邻位连接法构建遗传进化树,bootstrap检验1 000次;以Kimura 2参数方法计算基因型间遗传距离。结果从2008—2020年4 378份四川省AFP病例标本中分离鉴定到13株CVB3,从2013—2019年四川省454份非EV 71和CVA16感染的HFMD病例标本中分离鉴定到1株CVB3。14株CVB3四川分离株之间核苷酸同源性为84.4~98.8%,氨基酸同源性为84.1~100.0%。14株CVB3均属于G基因型,其中11株在G1基因亚型,3株在G2基因亚型。结论四川省存在CVB3病毒的多条传播链,流行趋势呈现周期性变化;G1、G2两种基因亚型在四川省同时流行,G1基因亚型为四川省CVB3的优势基因亚型。Objective To understand the genetic characteristics of Coxsackievirus B3(CVB3) isolated from cases of acute flaccid paralysis(AFP) and hand-foot-and-mouth disease(HFMD), and to explore its mutation pattern and transmission dynamics in Sichuan Province. Methods Specimens of AFP cases from 2008—2020 and HFMD cases infected with non-enterovirus 71(Enterovirus 71,EV71) and Coxsackie virus A16(CVA16) from 2013—2019 were subjected to cell isolation and culture,serological identification,determination of the full VP1 gene sequence and nucleotide. The genetic evolution tree was constructed by using neighbor joining method with MEGA6.0 software,and bootstrap test was performed 1 000 times;the genetic distance between genotypes was calculated by using Kimura 2 parameter method. Results Thirteen CVB3 strains were isolated and identified from 4 378 specimens of AFP cases in Sichuan Province from 2008 to 2020,and one CVB3 strain was isolated and identified from 454 specimens of non-EV-71 and CVA16-infected HFMD cases in Sichuan Province from 2013 to 2019. Nucleotide homology among the 14 CVB3 isolates ranged from 84.4 to 98.8%, and amino acid homology ranged from 84.1 to 100%. All 14 CVB3 strains belonged to the G genotype;specifically, 11 of them were in the G1 genotype and 3 in the G2 genotype. Conclusions Multiple transmission chains of CVB3 virus existed in Sichuan Province,and the epidemic trend showed periodic changes. Two genotypes, G1 and G2, were prevalent in Sichuan Province,and the G1 genotype was the dominant genotype of CVB3 in Sichuan Province.
