成人室管膜瘤MRI表现及ADC值与Ki67增殖指数相关性研究

摘　　要：目的:比较成人脑内室管膜和间变性室管膜瘤MRI表现及平均最小表观弥散系数(ADC)值,分析平均最小ADC值与Ki67指数相关性。方法:回顾性分析经病理证实的15例Ⅱ级室管膜瘤与12例Ⅲ级间变性室管膜瘤患者临床资料并测量ADC值,分析二者MRI表现及平均最小ADC值,评估平均最小ADC值与Ki67指数相关性。结果:瘤体实性部分平均最小ADC值在成人脑内Ⅱ、Ⅲ级室管膜瘤分级中差异有统计学意义(P<0.01),曲线下面积(AUC)为0.872;平均最小ADC值与Ki67指数呈负相关(r=-0.557,P=0.003)。结论:平均最小ADC值在成人脑内室管膜瘤分级中有一定的临床价值,可辅助反映瘤细胞增殖活性。Purpose: To compare the MRI findings and mean minimum apparent diffusion coefficient(ADC) value between adult intracranial ependymoma and anaplastic ependymoma, and to analyze the correlation between the mean minimum ADC value and Ki67 proliferative index. Methods: Fifteen cases of adult intracranial ependymoma and 11 cases of pathologically confirmed anaplastic ependymoma were analyzed retrospectively in our hospital. The differences of MRI findings and mean minimum ADC values between the two groups were compared,and the correlation of mean minimum ADC and Ki67 index expression was analyzed.Results: The mean minimum ADC value of the tumor solid part was with statistical difference between adult intracranial ependymoma grade Ⅱand Ⅲ(P<0.001). The area under the curve(AUC) value was 0.872, and the optimal cutoff minimum ADC value for differential diagnosis of the two tumors was 0.892×10;mm;/s, and the minimum ADC value and Ki67 index was with negative correlation(r=-0.557, P=0.003). Conclusion: The mean minimum ADC value has clinical value in grading diagnosis among adult interstitial ependymoma and ependymoma, and can reflect tumor cell proliferation activity.
