摘　　要：目的:探讨胰腺局灶性脂肪浸润的CT及MRI典型表现。方法:回顾性分析胰腺局灶性脂肪浸润的患者30例,其中22例行CT检查,8例行MRI检查。结果:①病灶位置:累及胰头13例,胰头及钩突4例,胰头及胰颈部3例,胰颈部1例,胰体颈交界1例,胰体部5例,胰尾部3例。②病灶CT表现:胰腺局灶性脂肪浸润在CT平扫均表现为低密度,CT值为-115.0~-13.0HU,增强扫描未见明显强化。③病灶MRI表现:于T2WI压脂序列,1例与胰腺呈等信号,1例呈稍高信号,6例呈低信号;于T1同相位,1例与胰腺呈等信号,7例呈稍高信号,8例病灶于T1反相位均表现为信号衰减,与周围胰腺实质相比呈明显低信号;于DWI序列,8例病灶均未见明显高信号;增强扫描病变未见明显强化。结论:胰腺局灶性脂肪浸润发病部位多位于胰头及钩突前部,具有一定的影像特点,结合CT及MRI检查能够作出准确的定性诊断。Purpose: To explore the typical CT and MRI manifestations of the focal fatty infiltration of the pancreas.Methods: The image findings of 30 cases with focal fatty infiltration of the pancreas were retrospectively analyzed. Twenty-two patients underwent CT scan, 8 patients underwent MRI scan. Results:① Location of the lesion: 13 cases located at the head of the pancreas, 4 cases located at the head of the pancreas and uncinate process,3 cases located at the head and the neck of the pancreas, 1 case located at the neck of the pancreas, 1 case located at the junction of the body and neck of pancreas, 5 cases located at the body of the pancreas, 3 cases located at the tail of the pancreas.② CT manifestation: The focal fatty infiltration of pancreas showed low density on plain CT scan,CT value was about-115.0 HU to-13.0 HU, no obvious enhancement was found on enhanced CT scan.③ MRI manifestation: On T2WI,1 case showed equal intensity signal with pancreas,1 case showed slightly higher intensity signal, 6 cases showed low intensity signal;on the in-phase T1-weighted images, 1 case showed equal intensity signal with pancreas,7 cases showed slightly high intensity signal, in the opposed-phase gradient-echo images,8 cases showed signal intensity attenuation, which was significantly lower than the surrounding pancreas;on the DWI, 8 cases showed low signal, and no obvious enhancement was found on enhanced scan. Conclusions: The focal fatty infiltration of pancreas is mainly located in the head of pancreas and the anterior part of uncinate process, which has certain imaging characteristics. Combined with CT and MRI, accurate qualitative diagnosis can be made.
