摘　　要：目的:分析某医院首批国家组织药品集中采购和使用(国家集采)在第一个协议期的成效,总结经验,提高国家组织药品集中采购和使用的社会效益。方法:分别调取某医院2019年协议期与2018年同期共24个月的药品使用相关数据,结合国家政策文件要求及医院具体实施工作方案进行成效分析。结果:较上年度同期相比,实施国家集采后该院2019年协议期内药品费用(不含中药饮片)由37300.70万元降至34767.28万元,降低了2533.42万元,降幅为6.79%;中标药品品种总使用量从349.99万片(支)增至471.80万片(支),增加了121.81万片(支),增幅为34.80%;中标药品品种总使用金额从2355.27万元降至1026.37万元,降低了1328.90万元,降幅为56.42%;疗效近似的其他通用名药品金额从1902.69万元降至951.95万元,降低了950.74万元,降幅为49.97%。结论:实施国家集采可有效降低药品费用支出,使患者的诊疗过程更加经济化。医院药师可通过政策培训、数据监测和合理用药管控等方式在国家集采政策执行过程中发挥积极作用。Objective:To analyse the implementation effect of the first batch of pilot project of national organized centralized drug procurement and application in the first contract year in a tertiary-grade class a hospital,and to sum up the experience,so as to imporve the social benefits of national organized centralized drug procurement and application.Methods:Data of drug application of the 2019 contract period and the same period in 2018 for a total of 24 months in this hospital,combined with the national policy documents and specific implementation plan of hospital to analyse the implementation effect.Results:Compared with the same period in 2018,after the implementation of national centralized procurement,drug expenditure(except traditional Chinese medicine decoction pieces)in this hospital had decreased from 373.0070 million yuan to 347.6728 million yuan,with a decrease of 25.3342 million yuan and decreasing amplitude of 6.79%;the total usage amount of bid-winning drugs had increased from 3.4999 million tablets(pieces)to 4.7180 million tablets(pieces),with an increase of 1.2181 million tablets(pieces)and an increasing amplitude of 34.80%;the total consumption sum had decreased from 23.5527 million yuan to 10.2637 million yuan,with a decrease of 13.2890 million yuan and decreasing amplitude of 56.42%;sum of money of other generic drugs with similar efficacy had decreased from 19.03 million yuan to 9.52 million yuan,with a decrease of 9.51 million yuan and decreasing amplitude of 49.97%.Conclusion:The implementation of national centralized procurement policy can effectively reduce drug expenditure and also make the patient's diagnosis and treatment process more economical.Hospital pharmacists can play a positive role in the implementation process of national organized centralized drug procurement and application,by means of policy training,data monitoring and rational medication controlling.
