ICU肺癌术后患者家属疾病不确定感状况及其影响因素的现况研究

摘　　要：目的探讨ICU肺癌术后患者家属的疾病不确定感状况及其影响因素。方法采用便利抽样法,使用自行设计的一般资料调查表、中文版疾病不确定感家属量表、状态焦虑量表和社会支持评定量表对2019年3月至2019年10月本院160例肺癌术后ICU患者家属进行问卷调查。结果肺癌术后ICU患者家属疾病不确定感得分为(68.22±12.13)分;疾病不确定感4个维度中,不可预测性维度得分最高;疾病不确定感总分及各维度得分均与状态焦虑得分呈正相关,其中疾病不确定感的复杂性维度得分与主观支持得分呈负相关(均P<0.05);多重线性回归分析显示,患者家属的状态焦虑和年龄是ICU患者家属疾病不确定感的影响因素(均P<0.05)。结论 ICU肺癌术后患者家属疾病不确定感较严重,其状态焦虑得分越高,疾病不确定感则越高。在临床工作中护士要关注ICU肺癌术后患者,还应当重视其年龄大和焦虑严重的患者家属,明确患者家属的需求,及时与家属沟通,从而降低家属疾病不确定感。Objective To investigate uncertainty about the disease and anxiety of families of the patients staying in the ICU after lung cancer surgery and the social support to the patients, family members as well as to explore the correlations between the influencing factors and the uncertainty about the disease. Methods Family members of 160 lung cancer patients in the ICU between March and October 2019 were enrolled in the investigation with convenience sampling. General information questionnaire,the Disease Uncertainty Scale for Patients’ Families(Chinese Version),the State Anxiety Inventory(SAI), and the social support rating scale(SSRS)were used on the day of the operation. Results The score of the disease uncertainty was 68.22±12.13. Among its four dimensions,unpredictability was scored as the highest,followed by uncertainty,complexity, and lack of information. The total score and the scores of all the dimensions were positively associated with the score of stateanxiety(P<0.05). The score of the dimension of complexity was negatively correlated with that of the dimension of subjective support(P<0.05). The state anxiety and age of the family members were the influencing factors of the disease uncertainty(P<0.05). Conclusions The disease uncertainty of families of lung cancer patients in the ICU after surgery remains at a higher level. The more serious the stateanxiety is,the more evident of the disease uncertainty. Therefore,nurses should not only care for the lung cancer patients in the ICU after surgery,but it is also important to pay attention to the elderly family members and the family members with serious anxiety. Nurses should clearly understand the requirement of patients,timely communicate with family members,thereby to reduce their uncertainty of the disease.
