摘　　要：目的探索早中期肝癌患者的创伤后成长心理轨迹,为制订针对性的心理护理干预措施提供参考依据。方法2020年10月—2021年3月选取重庆市某三级甲等医院感染科住院并接受简体中文版创伤后成长评定量表调查的早中期肝癌患者作为研究对象,以THRIVE模型为框架,采用质性研究中的现象学方法,对14例早中期原发性肝癌患者进行半结构式深入访谈,并运用Colaizzi7步分析法进行资料分析。结果归纳出6个主题:感知危机时负性情绪强烈;有效调动内外资源,获得希望感;发展成长型思维,讲述新故事;发现正向变化,积极应对;欣赏生活,寻找生命意义;感悟人生,以实际行动表明成长。结论早中期原发性肝癌患者的创伤后成长具有动态变化发展的特点,医护人员应及时关注早中期肝癌患者不同阶段的心理变化,并采取针对性措施促进其创伤后成长,以增进其心理健康,提高生活质量。Objective To explore the psychological trajectory of post-traumatic growth in early and middle hepatocellular carcinoma patients and to provide reference for the formulation of targeted psychological nursing intervention measures according to the test.Methods From October 2020 to March 2021,patients with early and mid-stage liver cancer who were hospitalized in a GradeⅢClass A hospital's department of infection in Chongqing were selected as the study subjects.The THRIVE model was used as the framework and phenomenological methods of qualitative research were adopted.A semi-structured in-depth interview was conducted in 14 patients with early and middle stage primary liver cancer,and data were analyzed by Colaizzi7 step analysis method.Results 6 themes were summarized:strong negative emotions during crisis perception,eff ective mobilization of internal and external resources to achieve a sense of hope,develop a growth mindset and tell new stories,fi nd positive changes and actively respond,appreciate life and looking for the meaning of life,feel life and show growth with practical action.Conclusion The post-traumatic growth of patients with early-intermediate primary liver cancer is characterized by dynamic change and development.Medical staff should pay attention to the psychological changes of patients with early-intermediate primary liver cancer at diff erent stages in time,take targeted measures to promote their post-traumatic growth,so as to improve their mental health and quality of life.
