摘  要:目的:探讨类风湿关节炎(rheumatoid arthritis,RA)患者与原发性干燥综合征(primary Sjögren's syndrome,pSS)患者肠道菌群的组成结构及评估肠道微生物群组成的差异性。方法:采集2019年1月至2020年3月我院收治的14例RA患者和14例pSS患者的晨起粪便。收集每份粪便样品的细菌总DNA,PCR扩增,应用Illumina hiSeq 2500高通量测序技术对16S rDNA基因的V3~V4区进行测序,得出2组样本的菌群生物信息数据,然后结合文献智库信息对得出的数据进行组间OTU分析、菌群结构多样性分析以及差异分析。结果:利用Shannon指数等绘制稀释曲线,并进行数据复杂度分析,结果显示所测数据量完整,可以反映研究对象中所有微生物的多样性信息。菌群多样性比较:2组α多样性指数分析中ACE指数、Chao1指数、Shannon指数、Simpson指数比较差异无统计学意义(P>0.05);菌群结构比较:序列分析结果显示2组在门水平和属水平上的主要菌门和主要菌属上差异无统计学意义(P>0.05);菌群结构的差异性分析:高维生物标识和基因组特征分析显示2组菌群差异有统计学意义(P<0.05)。根据LDA SCORE(effect size)大小,RA组核心菌群是Proteobacteria门、Bifidobacterium-adolescontis种,pSS组核心菌群是Bacteroidetes门、Bacteroidia纲、Bacteroidales目。结论:2种疾病肠道菌群具体种类上存在差异性。Objective:To investigate the composition of fecal microbiota in patients with rheumatoid arthritis(RA)and primary Sjögren's syndrome(pSS)and evaluate the difference of fecal microflora composition.Methods:Fecal samples were collected from 14 RA patients and 14 pSS patients.The total bacterial DNA from each fecal sample was collected,followed by PCR amplification.Illumina hiSeq 2500 high-throughput sequencing was used to sequence the v3-v4 region of 16S rDNA gene,and the biological information of the intestinal flora of the two groups were obtained.Then,combined with the information mining of literature think tanks,the intergroup OTU analysis,structural diversity analysis,and significant difference analysis were conducted.Results:The Shannon index was used to draw the dilution curve.The complexity analysis showed that the measured data were complete and could reflect the diversity information of all microorganisms in the research object.There was no significant difference between the two groups in ACE index,Chao1 index,Shannon index,and Simpson index in the analysis ofαdiversity index(P>0.05).Sequence analysis showed that there was no significant difference between the two groups at phylum and genus level(P>0.05).Analysis of high-dimensional biomarkers and genomic characteristics showed that the difference of flora between the two groups was statistically significant(P<0.05).According to the LDA SCORE(effect size),the core flora of RA group were Proteobacteria and Bifidobacterium-adolescentis,the core flora of pSS group were Bacteroidetes,Bacteroidia andBacteroidales.Conclusion:There are differences in the specific types of intestinal flora between the two diseases.
