基于数据挖掘的杨洪涛教授治疗特发性膜性肾病用药经验初探

摘　　要：目的:探究杨洪涛教授治疗特发性膜性肾病(IMN)用药规律。方法:收集稳定就诊于杨洪涛教授IMN专病门诊治疗经肾穿刺活检病理诊断为IMN患者医案,经纳排及中药名称标准化处理后提取处方内容。采用描述性统计法分析药物功效、性味及归经并结合R软件进行关联规则和聚类分析对组方用药进行数据挖掘。结果:共纳入41例患者资料,提取325首处方,涉及179味药物;提取常用药物组合20项,核心网络组合1项;60味高频药物总频数占总比>80%,可分为10个聚类方。结论:杨洪涛教授辨治IMN为本虚标实之证,临证多从风论治,整体予祛风通络、健脾益气、活血化瘀为法,兼顾渗湿泄浊、清利湿热、凉血止血等,后期予温补肝肾、固护肾精、滋阴养血以培护根本。Objective:To analyze the prescriptions and herbs of idiopathic membranous nephropathy(IMN)used by Hongtao Yang.Methods:Collecting the medical case of outpatients in professor Yang’s specialized disease Medical Clinic,who was diagnosed as IMN by renal biopsy.The prescriptions were selected with standardization of the Chinese medicine names.Descriptive statistics were used to analyze the efficacy,taste and meridian attribution of the drug and combined with R project for association rules and cluster analysis.Results:A total of 41 patients were included,325 prescriptions were selected,involving 179 drugs;20 common drug couples,and 1 core network combination were extracted;60 high utilization frequency drugs,whose frequency is over 80%of the total frequency,which can be divided into 10 clustering prescriptions.Conclusion:Professor Yang treats IMN,a deficiency as primary and excess as secondary syndrome,often based on the wind to design treatment clinically.Overall,promoting the spleen qi,dispelling wind and dreging collateral,and promoting blood circulation and remove blood stasis as well as discharging damp and turbid factor,clearing damp and heat,cooling blood to stop bleeding,and etc.finally,the primary treatments are warming and invigorating liver and kidney,relieving seminal emission,and nourishing yin and blood.
