无创胸液水平预测急性百草枯中毒早期肺损伤程度的研究

摘　　要：目的:通过检测无创胸液水平(thoracic fluid content,TFC)变化,全面了解百草枯中毒早期患者肺部损伤程度,有助于指导临床治疗,便于医师制定相关治疗方案。方法:观察本院2018年1月—2019年9月急诊医学科收治的53例急性百草枯中毒患者。使用心阻抗血流动力学监护系统(Impedance Cardiography Hemodynamic Monitors,Lifegard ICG)完成对TFC的检测,同时联合胸部CT检查结果评价肺部损伤评分,分析相关性。结果:发病4 h时,患者TFC与肺损伤评分在分别为(37.94±8.30)/kohm、(0.42±0.719)分,(r=0.542,P<0.001);72 h时分别为(72.58±15.32)/kohm、(8.70±3.38)分,(r=0.805,P<0.001)。结论:TFC与与中毒早期肺损伤程度呈现为正相关,无创TFC的检测结果可预测急性百草枯中毒早期肺损伤程度。Objective To evaluate the value of noninvasive thoracic fluid content(TFC)determination in the early lung injury assessment of paraquat poisoning.Methods The 53 patients with acute paraquat poisoning admitted to the Department of Emergency Medicine of our hospital from January 2018 to September 2019 were selected as the study subjects.They were all males,aged between 25 and 45 years old,with no history of lung disease,and the plasma concentration of paraquat within 4 hours of onset was 1~3μg/mL.To use Impedance Cardiography Hemodynamic Monitors(Lifegard ICG)for TFC measurement(onset 4 h,72 h),combined with chest CT evaluating(onset 4 h,72 h)lung injury score for early lung injury after poisoning and the degree and their relevance.Results TFC and lung injury score were(37.94±8.30)/kohm,(0.42±0.719),(r=0.542,P<0.001)at 4 h and(72.58±15.32)/kohm,(8.70±3.38),(r=0.805,P<0.001)at 72 h.Conclusion TFC was positively correlated with the degree of lung injury in early stage of poisoning.Noninvasive TFC determination has certain predictive value for the degree of lung injury in acute paraquat poisoning.
