带状疱疹后遗神经痛发生的影响因素分析

摘　　要：目的:分析带状疱疹后遗神经痛(postherpetic neuralgia,PHN)发生的影响因素,为患者治疗和干预方案优化提供指导。方法:选取2019年6月—2020年6月在南通大学第二附属医院接受治疗的145例带状疱疹患者,根据患者有无PHN发生情况,分为PHN组(n=54)和无PHN组(n=91)。通过单因素分析和Logistic回归分析影响患者发生PHN的风险因素。结果:单因素分析结果表明,年龄、皮损类型、急性期疼痛、皮损面积、发病到接受治疗时间,以及前驱痛、合并慢性疾病、C反应蛋白和红细胞沉降率水平异常均与PHN发生有关(P<0.05);Logistic回归分析显示,年龄、皮损类型、急性期疼痛、皮损面积、前驱痛、合并慢性疾病、红细胞沉降率水平异常是PHN发生的风险因素(P<0.05)。结论:带状疱疹患者年龄、临床体征、治疗和合并症情况,对PHN发生会产生影响,需要临床治疗中针对性防控相关因素,以此预防和控制PHN发生,改善带状疱疹患者预后。Objective To analyze the influencing factors of postherpetic neuralgia(PHN).Methods 145 patients with herpes zoster treated in the Second Affiliated Hospital of Nantong University from June 2019 to June 2020 were studied.According to the occurrence of PHN,they were divided into PHN group(n=54)and no PHN group(n=91).Univariate analysis and logistic regression analysis were used to identify the risk factors affecting the occurrence of PHN,so as to provide guidance for the optimization of patient treatment and intervention plan.Results Univariate analysis showed that age,lesion type,acute pain,lesion area,time from onset to treatment,prodromal pain,chronic diseases,abnormal levels of C-reactive protein and erythrocyte sedimentation rate were all risk factors affecting the occurrence of PHN(P<0.05);Logistic regression analysis showed that age,lesion type,acute pain,lesion area,time from onset to treatment,chronic diseases and abnormal erythrocyte sedimentation rate were all risk factors affecting the occurrence of PHN(P<0.05).Conclusion The age,clinical signs,treatment and complications of patients with herpes zoster will affect the occurrence of PHN.It is necessary to prevent and control relevant factors in clinical treatment,so as to prevent and control the occurrence of PHN and improve the prognosis of patients with herpes zoster.
