截石位上半身抬高角度影响输尿管镜碎石术治疗上段输尿管结石的清石率

摘　　要：目的分析截石位上半身不同抬高角度对输尿管镜碎石术治疗上段输尿管结石清石率的影响。方法收集2019年—2020年于清华大学第一附属医院行输尿管镜碎石术治疗上段输尿管结石患者的临床资料。按照患者上半身的抬高角度将患者分为0°、10°、20°、30°组,利用卡方检验对比不同组别的清石率,利用逻辑回归分析筛选影响清石率的因素。结果各组的基线资料相比较差异无统计学意义(P>0.05)。20°和30°组的清石率高于0°和10°组的清石率(P<0.05),20°和30°组的清石率差异无统计学意义(P>0.05),0°组和10°组的清石率差异无统计学意义(P>0.05)。多因素分析表明结石距离肾盂输尿管连接部的距离和上半身抬高角度是影响清石率的相关因素。结论截石位时将上半身抬高至合适的倾斜角度可以提高术后清石率。Objective To analyze the effects of different incline angles of upper body with lithotomy position on the stone-free rate during ureteroscopic lithotripsy for patients with upper ureteral stones.Methods Clinical data of patients with upper ureteral stones cured by ureteroscopic lithotripsy during 2019 and 2020 in our hospital were collected.The patients were allocated into 0°group,10°group,20°group and 30°group by the incline angles of upper body with lithotomy position.The stone-free rates were compared with Chi-square test.The factors affecting the stone-free rates were screened with Logistic regression analysis.Results There were no difference in the baseline data among the groups(P>0.05).The stone-free rates were higher in the 20°group and 30°group than in the 0°group and 10°group(P<0.05),but there was no difference between the 20°group and 30°group,nor between the 0°group and 10°group(P>0.05).The incline angle and distance between stone and ureteral pelvic junction were identified as the factors affecting stone-free rate.Conclusion Raising the incline angle to an appropriate scale during ureteroscopic lithotripsy will be helpful in increasing the stone-free rate.
