老年全髋关节置换术患者居家护理需求及影响因素

摘　　要：目的调查老年全髋关节置换术(THR)患者的居家护理需求现状,分析其影响因素,为制定针对性延续护理方案提供依据。方法选取2019年6月至2020年7月在南海区第四人民医院行THR治疗的住院老年患者,应用一般资料收集表、居家护理需求量表、生活自理能力量表和社会支持评定量表进行调查,采用单因素分析和多元线性回归分析法分析THR老年患者居家护理需求的影响因素。结果共计派发问卷400份,回收有效问卷384份,有效回收率为96.0%。THR老年患者的居家护理需求平均得分为(18.4±5.3)分,需求率为57.6%;其中康复护理需求得分最高,平均为(4.5±1.3)分,需求率为63.8%。经多因素回归分析可知,文化程度、术后时间、并发症情况、是否独居、生活自理能力和社会支持水平均为THR老年患者居家护理需求的独立影响作用(R^(2)=0.773,P<0.05)。结论THR老年患者对居家护理的需求较高,临床医护人员应针对患者的个体评估情况,提供个体化的护理干预。Objective To investigate the current situation of home care needs of elderly patients with total hip replacement(THR)and analyze the influencing factors,so as to provide the basis for formulating targeted continuing nursing programs.Methods From June 2019 to July 2020,elderly patients undergoing THR in the Fourth People′s Hospital of Nanhai District were selected.The general data collection table,home care demand scale,self-care ability scale and social support rating scale were used to investigate.The influencing factors of home care needs of elderly patients with THR were analyzed by single factor analysis of variance and multiple linear regression analysis.Results A total of 400 questionnaires were distributed and 384 valid questionnaires were recovered,and the effective recovery rate was 96.0%.The average score of home care demand of elderly patients with THR was(18.4±5.3)points,and the demand rate was 57.6%;the score of rehabilitation care demand was the highest(4.5±1.3),and the demand rate was 63.8%.The results of multivariate regression analysis showed that education level,postoperative time,complications,living alone,self-care ability and social support level were independent influence factors(R^(2)=0.773,P<0.05).Conclusion The elderly patients with THR have a high demand for home care,so the clinical medical staff should pay attention to the individual assessment of patients of patients to provide individualized intervention care.
