摘 要:目的:基于Toll样受体4(toll like receptor 4,TLR4)/核因子-κB(nuclear factor kappa B,NF-κB)炎性轴探讨针灸联合感觉运动训练对膝骨关节炎(knee osteoarthritis,KOA)大鼠运动能力的改善作用。方法:50只SD大鼠随机取10只作为空白对照组,另40只大鼠采用向关节腔注射经典乙酸钠复制KOA模型,然后将造模成功的36只大鼠按随机体质量排序法分为模型组、感觉运动训练组、针灸组、针灸联合感觉运动训练组,每组各9只。检测各组大鼠步态评分、爪压评分、软骨组织病理学、Mankin′s评分;ELISA法检测大鼠膝骨关节软骨白细胞介素-6(interleukin-6,IL-6)、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)水平;Wstern Blot法检测大鼠膝骨关节软骨TLR4、NF-κB蛋白表达;RT-PCR法检测关节软骨TLR4 mRNA、NF-κB mRNA相对表达量。结果:与模型组比较,感觉运动训练组、针灸组、针灸联合感觉运动训练组步态评分、爪压评分、Mankin′s评分显著降低(P<0.05),同时可显著降低膝关节软骨IL-6、TNF-α水平和TLR4 mRNA、NF-κB mRNA相对表达量及TLR4、NF-κB蛋白相对表达量(P<0.05),且针灸联合感觉运动训练组<针灸组<感觉运动训练组。结论:针灸联合感觉运动训练可改善KOA大鼠运动能力,减轻炎症反应、软骨损伤,机制可能与调控TLR4/NF-κB炎性轴有关。Objective:To investigate the effect of acupuncture combined with sensorimotor training on motor ability of Knee osteoarthritis(KOA)rats based on the Toll-like receptor 4(TLR4)/Nuclear factor-kappa B(NF-κB)inflammatory axis.Methods:Ten of 50 SD rats were randomly selected as the blank control group,and the other 40 rats were injected with classic sodium acetate into the joint cavity to replicate the KOA model.Then,the 36 rats successfully modeled were divided into the model group,sensorimotor training group,acupuncture group,acupuncture combined sensorimotor training group according to random bodyweight ranking method,with 9 rats in each group.Detect the gait score,paw pressure score,cartilage histopathology,Mankin′s score of each group of rats.ELISA method was used to detect the levels of interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)in the rat knee joint cartilage.Western Blot method was used to detect the expression of TLR4 and NF-κB protein in the articular cartilage of the rat knee.And the relative expression of TLR4 mRNA and NF-κB mRNA in articular cartilage was detected by the RT-PCR method.At the same time,it can significantly reduce the levels of IL-6 and TNF-αin knee cartilage,the relative expression of TLR4 mRNA and NF-κB mRNA and the relative expression of TLR4 and NF-κB protein(P<0.05),and the acupuncture combined with sensorimotor training group<Acupuncture group<Sensorimotor training group.Results:Compared with the model group,the gait score,paw pressure score,and Mankin′s score of the sensorimotor training group,acupuncture,and moxibustion combined with sensorimotor training group were significantly reduced(P<0.05)Conclusion:Acupuncture combined with sensorimotor training can improve motor performance and reduce the inflammatory response and cartilage damage in KOA rats,which may be related to the regulation of TLR4/NF-κB inflammatory axis.
