保肝利水汤治疗阴虚水停型原发性肝癌腹水临床观察

摘　　要：目的探讨保肝利水汤对阴虚水停型原发性肝癌腹水患者的临床疗效。方法筛选2019年11月—2020年6月于大连市第六人民医院就诊的阴虚水停型原发性肝癌腹水患者64例,随机分为2组。其中对照组32例予常规西医治疗,治疗组32例加用保肝利水汤治疗,疗程为14 d。观察2组患者中医证候积分改变情况,治疗前后患者肝功能改善情况,并比较2组临床疗效。结果治疗后,2组中医证候积分及肝功能指标总胆红素(TBIL)、谷草转氨酶(AST)、谷丙转氨酶(ALT)均降低(P<0.05),且治疗组改善更显著(P<0.05);对照组总有效率为71.88%(23/32),治疗组总有效率为90.63%(29/32),治疗组临床疗效更佳(P<0.05)。结论保肝利水汤能够改善患者肝功能,缓解患者腹水、腹胀、胁肋疼痛等症状,发挥理气疏肝、消肿利水之效,临床可有效治疗阴虚水停型原发性肝癌腹水。Objective To discuss the clinical curative effect of Baogan Lishui decoction in the treatment of ascites of primary liver cancer with yin deficiency and water stagnation.Methods Sixty-four patients with ascites of primary liver cancer treated in Dalian Sixth People’s Hospital from November 2019 to June 2020 were selected and randomly divided into 2 groups.Among them,32 cases in the control group were treated with conventional western medicine,and 32 cases in the treatment group were treated with Baogan Lishui decoction for 14 days.The changes of traditional Chinese medicine syndrome scores and the improvement of liver function before and after treatment were observed,and the clinical efficacy of the two groups was compared.Results After treatment,traditional Chinese medicine syndrome score and liver function indexes of total bilirubin(TBIL),AST and ALT were all reduced in both groups(P<0.05),and the improvement was more significant in the treatment group(P<0.05).The total effective rate of the control group was 71.88%(23/32),and the total effective rate of the treatment group was 90.63%(29/32).The clinical efficacy of the treatment group was better(P<0.05).Conclusion Baogan Lishui decoction can improve the liver function of patients,relieve symptoms such as ascites,abdominal distension and rib pain,exert the effect of regulating qi and soothing liver,reducing swelling and benefiting water,and effectively treat primary liver cancer ascites caused by yin deficiency and water stagnation.
