益气养心汤联合西药治疗慢性心力衰竭患者的效果

摘　　要：目的:探讨益气养心汤联合西药治疗慢性心力衰竭患者的效果。方法:选取71例气虚血瘀型慢性心力衰竭患者为研究对象,采用随机数字表法分为对照组35例和观察组36例。对照组给予常规西药[马来酸依那普利片+卡维地洛片+托拉塞米分散片]治疗,观察组在对照组基础上加用益气养心汤治疗,疗程2个月。比较两组心肌酶谱指标[肌酸激酶(CK)、肌酸激酶同工酶(CK-MB)、α-羟丁酸脱氢酶(α-HBDH)、乳酸脱氢酶(LDH)]水平、血管内皮功能指标[内皮素(ET)、血管紧张素Ⅱ(AngⅡ)、一氧化氮(NO)]水平、明尼苏达心力衰竭生命质量调查表(MLHFQ)评分及不良反应发生率。结果:治疗2个月后,观察组CK、CK-MB、α-HBDH、LDH、ET、AngⅡ水平及MLHFQ评分均低于对照组,NO水平高于对照组,差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:益气养心汤联合西药治疗慢性心力衰竭患者,可有效改善患者心肌酶谱指标水平,促进患者血管内皮功能恢复,提高其生命质量,效果优于单用西药。Objective:To explore effects of Yiqi Yangxin decoction in adjuvant treatment of patients with chronic heart failure treated by western medicine.Methods:71 patients with chronic heart failure of Qi deficiency and blood stasis type were selected as the research objects,and were divided into control group(35 cases)and observation group(36 cases)by using the random number table method.The control group was treated with conventional western medicine[Enalapril maleate tablets+Carvedilol tablets+Torasemide dispersible tablets],while the observation group was treated with Yiqi Yangxin decoction on the basis of that of the control group.The course of treatment was 2 months.The myocardial enzyme levels[creatine kinase(CK),creatine kinase isoenzyme(CK-MB),α-hydroxybutyrate dehydrogenase(α-HBDH),lactate dehydrogenase(LDH)],the vascular endothelial function index levels[endothelin(ET),angiotensin II(Ang II),nitric oxide(NO)],the Minnesota living with heart failure questionnaire(MLHFQ)score and the adverse reactions were compared between the two groups.Results:2 months after the treatment,the levels of CK,CK-MB,α-HBDH,LDH,ET and Ang II and the MLHFQ score of the observation group were lower than those of the control group;the NO level was higher than that of the control group;and the differences were statistically significant(P<0.05).However,there was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions:Yiqi Yangxin decoction combined with western medicine has a good effect in the treatment of chronic heart failure,which can effectively improve the myocardial enzyme spectrum levels,promote the recovery of the vascular endothelial function,and improve the quality of life.Moreover,it is superior to single western medicine treatment.
