针刺联合参苓白术汤辅助西药治疗脾虚型功能性腹泻患者的效果

摘　　要：目的:探讨针刺联合参苓白术汤辅助西药治疗脾虚型功能性腹泻(FD)患者的效果。方法:回顾性分析2019年4月至2021年4月该院收治的150例FD患者的临床资料,根据治疗方法不同将患者分为对照组和观察组各75例。两组均给予常规西药治疗,在此基础上,对照组予以参苓白术汤治疗,观察组在对照组基础上加用针刺治疗(选取足三里、脾俞、天枢、关元及中脘穴),连续治疗4周。比较两组临床疗效、胃肠激素[胃动素(MTL)、血管活性肠肽(VIP)和胃泌素(GAS)]水平、炎性因子[肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)和超敏C反应蛋白(hs-CRP)]水平及不良反应发生率。结果:观察组治疗总有效率为97.33%(73/75),高于对照组的88.00%(66/75),差异有统计学意义(P<0.05);治疗后,观察组VIP、MTL、GAS、TNF-α、IL-6和hs-CRP水平均低于对照组,差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:在常规西药治疗基础上,采用针刺联合参苓白术汤治疗脾虚型FD患者可获得理想效果,利于改善胃肠功能,减轻炎症反应,安全可靠,效果优于单用参苓白术汤。Objective:To explore effects of acupuncture combined with Shenling Baizhu decoction in treatment of patients with functional diarrhea(FD)of spleen deficiency.Methods:The clinical data of 150 FD patients admitted to our hospital from April 2019 to April 2021 were retrospectively analyzed.According to different treatment methods,these patients were divided into control group and observation group,75 cases in each group.Both groups were given conventional western medicine treatment.On this basis,the control group was treated with Shenling Baizhu decoction,while the observation group was treated with acupuncture(Zusanli,Pishu,Tianshu,Guanyuan and Zhongwan points).The treatments continuously lasted for 4 weeks.The clinical efficacy,the gastrointestinal hormone levels[motilin(MTL),vasoactive intestinal peptide(VIP)and gastrin(GAS)],the inflammatory factor levels[tumor necros is factorα(TNF-α),interleutin-6(IL-6)and high-sensitivity C-reactive protein(hs-CRP)]and the occurrence of adverse reactions were compared between the two groups.Results:The total effective rate of the observation group was 97.33%(73/75),which was higher than 88.00%(66/75)of the control group,and the difference was statistically significant(P<0.05).After the treatment,the levels of VIP,MTL,GAS,TNF-α,IL-6 and hs-CRP in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).However,there was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions:On the basis of the conventional western medicine,acupuncture combined with Shenling Baizhu decoction can achieve the ideal results in the treatment of the patients with FD of spleen deficiency,which is beneficial to improve the gastrointestinal function,reduce the inflammation reactions,and is safe and reliable.Moreover,it is superior to single Shenling Baizhu decoction.
