手法闭合性复位治疗急性创伤性桡骨远端骨折的临床探讨

摘　　要：目的探讨手法闭合性复位治疗急性创伤性桡骨远端骨折的临床效果。方法随机选取2018年7月—2019年7月100例创伤性挠骨远端骨折患者资料进行回顾分析,对照组(50例)行常规切开复位内固定术治疗,观察组(50例)行手法闭合性复位术治疗,比较手术恢复情况、恢复效果、疼痛程度、功能状态、疗效、预后情况。结果观察组手术用时(22.04±2.81)min、消肿用时(3.10±0.32)d、骨折愈合用时(44.20±5.15)d均短于对照组,干预后VAS(2.30±0.41)分低于对照组,差异有统计学意义(t=31.439、20.484、16.950、35.246,P<0.05);干预后Barthel指数(87.81±4.05)分、优良率94%(47/50)均高于对照组,差异有统计学意义(t=30.082,χ^(2)=6.363,P<0.05);并发症发生率2%(1/50)低于对照组,差异有统计学意义(χ^(2)=4.396,P=0.036)。结论手法闭合性复位术能够缩短手术时间,改善手术效果,减轻疼痛程度,促进血肿消除,减少感染等并发症,使骨折尽快愈合,使患者的日常生活活动能力得到提升,达到改善预后的目的。Objective To investigate the clinical effect of manual closed reduction in the treatment of acute traumatic distal radius fractures.Methods A retrospective analysis of the data of 100 patients with traumatic distal flexible bone fractures who were randomly selected from July 2018 to July 2019.The control group(50 cases)underwent conventional open reduction and internal fixation,and the observation group(50 cases)underwent manipulation closed reduction surgery treatment,compared the operation and recovery,recovery effect,pain degree,functional status,curative effect,and prognosis.Results The operation time(22.04±2.81)min,detumescence time(3.10±0.32)d and fracture healing time(44.20±5.15)d in the observation group were shorter than those in the control group,after the intervention,the VAS(2.30±0.41)points was lower than that in the control group,the difference was statistically significant(t=31.439,20.484,16.950,35.246,P<0.05).After the intervention,the barthel Index(87.81±4.05)points and excellent and good rate 94%(47/50)in the observation group were higher than those in the control group,the difference was statistically significant(t=30.082,χ^(2)=6.363,P<0.05).The incidence of complications in the observation group was 2%(1/50)and lower than that in the control group,the difference was statistically significant(χ^(2)=4.396,P=0.036).Conclusion Manual closed reduction can shorten the operation time,improve the effect of the operation,reduce the degree of pain,promote the elimination of hematoma,reduce the complications such as infection,make the fracture heal as soon as possible,improve the patient's activities of daily living,and achieve the purpose of improving the prognosis.
