摘 要:目的研究中药清开灵注射液用于延长血小板体外保存期的代谢和形态变化,探索清开灵延长血小板体外保存期的可行性。方法单采血小板中添加终浓度1%清开灵注射液的实验组保存到d1、d3、d5、d8、d10和d14(6个时间点)和不添加清开灵注射液的对照组保存到d1、d3和d5(3个时间点)抽取标本,检测在(22±2)℃振荡保存下的血小板计数、形态学积分、生化指标、pH和低渗休克反应率。结果1)实验组d14内和对照组5 d内,血小板计数无明显变化(P>0.05)。保存5 d内实验组和对照组的血小板平均体积、血小板体积分布宽度值分别在不同保存时间点的组间比较都增大(P<0.05)。2)实验组14 d内和对照组5 d内血小板形态学积分值均减小(P<0.05),但保存8 d内,对照组的血小板形态学积分值减小幅度大于实验组的,特别在d8时差异显著(P<0.05)。3)实验组保存14 d内的葡萄糖、乳酸脱氢酶、乳酸、Na^(+)、K^(+);值均有不同程度的增高或降低(P<0.05),而Cl^(-)值在保存14d内无明显变化(P>0.05);对照组的葡萄糖、乳酸脱氢酶、乳酸、Na^(+)值保存5d内有显著变化(P<0.05),而K^(+)、Cl^(-)值无明显变化;保存5d内,对照组的葡萄糖消耗量明显比实验组大(P<0.05),乳酸脱氢酶、乳酸的生成量明显比实验组大(P<0.05),Na^(+)、K^(+)、Cl^(-)增加值与实验组比较不明显。4)实验组与对照组5 d内的pH值与d1相比均有降低,但对照组降低趋势明显(P<0.05),实验组则无明显变化(P>0.05)。实验组8d内低渗休克反应率变化不明显,对照组5 d内低渗休克反应率显著降低(P<0.05);保存d3、d5时,实验组的低渗休克反应率明显高于对照组(P<0.05)。结论添加1%清开灵注射液的单采血小板体外保存14 d内与不添加清开灵注射液的对照组5 d内的血小板计数、形态、生化指标、pH和低渗休克反应率比较,清开灵注射液至少可以延长血小板体保存期到10 d。Objective To study the metabolism and morphology changes of platelets in vitro using traditional Chinese medicine named Qingkailing injection as the additive solution,and to explore the viability of Qingkailing in the extension of platelet storage.Methods Apheresis platelets,adding 1%final concentration of Qingkailing injection,were taken as experiment groups,and sampled on 1,3,5,8,10 and 14 days(6 time points)of storage.Apheresis platelets without adding Qingkailing injection were taken as the control,and sampled on 1,3 and 5 days(3 time points)of storage.The platelet count,morphology scores,biochemical parameters,pH and response rate of hypotonic shock during agitated storage(22℃)were tested.Results 1)No significant change in platelet count was noticed in both experimental group(within 14 days)and the control(within 5 days)(P>0.05).The MPV and PDW of both groups increased at any sampling time within 5 days(P<0.05).2)The morphology score of experimental groups and the control all decreased within the storage period(P<0.05),but its decrease of the control was greater than that of the experimental groups,especially on day 8(P<0.05).3)Glucose,lactate dehydrogenase,lactate,Na^(+),and K^(+)values increased or decreased in varying degrees(P<0.05),while Cl^(-)value stayed almost the same during 14 days(P>0.05).Glucose,lactate dehydrogenase,lactate and Na^(+)value changed significantly in the control within 5 days(P<0.05),while K^(+)and Cl^(-)value did not(P>0.05).Within 5-day storage,the glucose consumption,lactate dehydrogenase and lactate generation in the control were significantly greater than those in experimental groups(P<0.05),but the added value of Na^(+),K^(+)and Cl^(-)showed no significant difference(P>0.05).4)pH value,relative to the baseline of day 1,decreased in both groups within 5 days,and its decreasing trend was significant in the control(P<0.05),but not in the experimental group(P>0.05).No significant difference was noticed in the response rate of hypotonic shock in experimental groups within 8 days
