摘  要:目的:研究经鼻间歇正压通气(NIPPV)下微创给予外源性肺表面活性物质(PS)治疗新生儿呼吸窘迫综合征(NRDS)的疗效。方法:纳入2019年4月到2021年6月在新余市人民医院接受治疗的120例呼吸窘迫综合征(RDS)新生儿作为研究对象,按照治疗方式的不同将其分为A组(NIPPV+外源性PS)、B组〔经鼻持续气道正压通气(NCPAP)+外源性PS〕、C组(气管插管+外源性PS)三组,每组40例。比较三组患儿治疗相关指标、治疗前后血气分析结果及并发症发生率。结果:A、C组新生儿PS注射次数显著少于B组,A组新生儿无创(或有创)通气时间、吸氧时间、住院时间显著短于B、C组,差异均具有统计学意义(P<0.05)。三组新生儿死亡率差异无统计学意义(P>0.05)。三组新生儿治疗后动脉血二氧化碳分压(PaCO_(2))、吸入氧浓度(FiO_(2))水平组内比较显著低于治疗前,氧合指数(OI)水平显著高于治疗前,差异具有统计学意义(P<0.05);治疗后,A组新生儿PaCO_(2)、FiO_(2)水平显著低于B、C组,OI水平显著高于B、C组,差异具有统计学意义(P<0.05)。A组新生儿并发症总发生率为10.00 %,显著低于B组、C组(27.50 %、35.00 %),差异具有统计学意义(P<0.05)。结论:NIPPV下微创给予外源性PS治疗NRDS具有良好效果。Objective To study the effect of minimally invasive Treatment of exogenous pulmonary surfactant (PS) under nasal intermittent positive pressure ventilation (NIPPV) on neonatal respiratory distress syndrome (NRDS).Methods A total of 120 newborns with respiratory distress syndrome (RDS) who were treated in Xinyu People’s Hospital from April 2019 to August 2021 were included as research objects.According to different treatment methods,they were divided into three groups:group A (NIPPV+PS),group B〔(NCPAP)+PS〕and group C (tracheal intubation+PS),with 40 cases in each group.The treatment-related indexes,blood gas analysis results before and after treatment and the incidence of complications were compared among the three groups.Results The number of PS injections of newborns in group A and C was significantly less than that in group B.The time of noninvasive(or invasive) ventilation,oxygen inhalation and hospital stay of newborns in group A were significantly shorter than those in group B and C,and the differences were statistically significant (P < 0.05).There was no significant difference in neonatal mortality among the three groups (P > 0.05).After treatment,the levels of arterial carbon dioxide partial pressure (PaCO_(2)) and inhaled oxygen concentration (FiO_(2)) in the three groups were significantly lower than those before treatment,and the oxygenation index (OI) level was significantly higher than that before treatment,and the difference was statistically significant (P < 0.05).After treatment,PaCO_(2)and FiO_(2)levels of newborns in group A were significantly lower than those in groups B and C,and OI levels were significantly higher than those in groups B and C,with a statistically significant difference (P < 0.05).The total incidence of neonatal complications in Group A was 10.00%,which was significantly lower than that in Group B and Group C (27.50%,35.00%),and the difference was statistically significant (P < 0.05).Conclusion Minimally invasive PS treatment of (NRDS) under NIPPV has a good effect.
