摘 要:目的:探讨二维斑点追踪成像(2D–STI)技术对糖尿病肾功能不全患者左室收缩功能的评估价值。方法:选取郑州市第一人民医院2017年11月至2019年12月收治的糖尿病肾功能不全患者65例作为观察组,另选取同期健康体检者65例作为对照组,两组均行2D–STI技术检查及超声心动图检查,比较超声相关参数、心外膜及心内膜心肌收缩期纵向峰值应变参数(LPSS)。结果:观察组患者的二尖瓣环舒张早期、晚期心肌运动速度比(Em/Am)、二尖瓣口舒张期血流速度E峰、A峰比(E/A)低于对照组健康体检者,差异具有统计学意义(P<0.05);观察组患者心内膜层及心外膜层的左室整体、基底段、心尖段、中间段心肌收缩期纵向峰值应变参数(LPSS)均低于对照组健康体检者,差异具有统计学意义(P<0.05)。结论:糖尿病肾功能不全患者Em/Am、E/A水平及各节段LPSS水平明显较正常人低,通过2D–STI技术检测可有效评估左室收缩功能,有助于临床早期诊断及预后评估。Objective To explore the application value of 2D-STI in the evaluation of the left ventricular systolic function in patients with diabetic renal insufficiency.Methods Sixty-five patients with diabetic renal insufficiency admitted to the First People’s Hospital of Zhengzhoul from November 2017 to December 2019 were selected as the observation group,and 65 healthy subjects in the same period were selected as the control group.Both groups received 2D-STI technical examination and echocardiography.The parameters related to ultrasound,the longitudinal peak strain parameters (LPSS) of myocardium and epicardium were compared.Results The myocardial velocity ratio (Em/Am) of early and late mitral annulus diastolic and the peak E and peak A ratio (E/A) of mitral orificium diastolic flow velocity in the observation group were lower than those in the control group,and the differences were statistically significant (P < 0.05).The longitudinal peak strain parameters (LPSS) of the whole left ventricle,basal segment,apical segment and middle segment of the myocardium in the observation group were lower than those in the control group,and the differences were statistically significant (P < 0.05).Conclusion Em/Am and E/A levels and LPSS levels in each segment of patients with diabetic renal insufficiency are significantly lower than those in normal subjects.2D-STI technique can effectively evaluate left ventricular systolic function,which is helpful for early clinical diagnosis and prognosis evaluation.
