基于虚、瘀、湿、毒理论探讨前列腺癌病机特点及西黄丸的运用

摘　　要：初步分析提炼前列腺癌及西黄丸相关文献,提出前列腺癌中医病机特点为"虚""瘀""毒""湿",治疗核心应为扶正和祛邪,临床应根据疾病的正邪盛衰特点调整侧重扶正或祛邪。西黄丸作为治疗恶性肿瘤的经典名方,功效上侧重于祛邪为主,且具有祛邪不伤正的药性特点,可适用于正气充盛或正不甚虚证型前列腺癌。综合现代药理研究进展,西黄丸可介导PI3K/Akt/mTOR信号通路,抑制PC3细胞生长,具有显著的抗肿瘤活性、改善前列腺微环境以及镇痛等作用,认为西黄丸对于前列腺癌患者可能产生较好治疗效果,以期为今后前列腺癌中医临床治疗提供新思路。Based on the preliminary analysis and extraction of prostate cancer and Xihuang Pill related literature, it is proposed that the TCM pathogenesis of prostate cancer is characterized by “deficiency”, “stasis”, “toxicity” and “dampness”, and the core of treatment should be strengthening vital Qi and eliminating pathogenic factor, and the clinical adjustment should focus on strengthening vital Qi or eliminating pathogenic factor according to the rise and fall of the disease. Xihuang Pill, as a modern classic prescription for the treatment of cancer diseases, mainly focuses on eliminating pathogenic factor, and has the property of eliminating pathogenic factor without damaging vital Qi, which can be applied to prostate cancer with excess of vital Qi or deficiency of vital Qi syndrome. Based on the research progress of modern pharmacology, Xihuang Pill can mediate PI3 K/Akt/mTOR signaling pathway, inhibit the growth of PC3 cells, has significant anti-tumor activity, improving prostate microenvironment and analgesic effects. It is considered that the Xihuang Pill for prostate cancer patients may have good treatment effect, in order to provide new ideas for the TCM clinical treatment of prostate cancer in the future.
