“柔肝缓急”法论治膀胱过度活动症

摘　　要：探讨“柔肝缓急”法用于膀胱过度活动症的辨治思维以及具体的临床运用。本病属于中医学上的“淋证”范畴,近年来有医家从肝论治膀胱过度活动症,采用了“镇肝息风”“疏肝健脾”“调肝理血”等治法。本文则认为此病病程长久,易反复发作可能与肝阴、肝血亏虚、湿热瘀滞有关。肝为刚脏,体阴而用阳,治疗上以柔肝缓急为要,以芍药甘草汤为基础方,酸甘化阴,柔肝缓急以息风、止痛、宁神、育阴利水,临床上加减辨证辅予清热利湿、活血化瘀之法。“柔肝缓急”法临床多用于治疗阴虚血亏的痉挛性疾病及各种疼痛等,异病同治,运用此法治疗膀胱过度活动症亦有奇效。现从病因病机入手,剖析其内涵以及临床具体应用,为临床上治疗本病提供了新的思路和方向。To explore the differentiation and treatment of bladder hyperactivity with the method of“soft liver and slow and urgent”and its specific clinical application.This disease belongs to the category of“drenching syndrome”in Chinese medicine.In recent years,some doctors have treated overactivity bladder from the liver,and adopted the treatment methods of“suppressing liver and soothing wind”“soothing liver and strengthening spleen”“regulating liver and regulating blood”and so on.In this paper,it is believed that the disease has a long course and is prone to recurrent attacks,which may be related to deficiency of liver Yin,liver blood and dampness-heat stasis.The liver is just zang,and the body is Yin but Yang is used.For treatment,soft liver and slow urgent is the key,Shaoyao Gancao decoction is used as the basic formula to sour and liquorice Yin and soft liver is used to soothe wind,relieve pain,calm the mind,nourish Yin and nourish water.Clinically,the method of clearing heat,reducing dampness,promoting blood circulation and removing blood stasis is dialectically supplemented.“Soft liver slow and urgent”method is mostly used in the treatment of Yin deficiency blood deficit spasmodic disease and various pain,different diseases,the same mechanism and syndrome type,the use of this method for the treatment of bladder hyperactivity is also a wonderful effect.This article starts with the etiology and pathogenesis,analyzes its connotation and clinical application,and provides a new way of thinking and direction for clinical treatment of this disease.
