中药熏洗对前交叉韧带重建术后康复影响的Meta分析

摘　　要：目的:系统评价中药熏洗对前交叉韧带重建术后康复的影响。方法:计算机检索PubMed、EMbase、The Cochrane Library、Web of Science、CNKI、CBM、WanFang和维普数据库,搜集对前交叉韧带重建术后康复影响的随机对照试验(RCT),检索时限均从建库至2020年11月30日。2名研究者独立进行文献筛选、资料提取、偏倚风险评价后,采用RevMan 5.3软件进行系统评价。结果:共纳入研究6项,Meta分析结果显示,前交叉韧带重建术后采用中药熏洗,能提高术后3月的Lysholm评分[MD=3.27,95%CI(1.97,4.56),P<0.00001],提高术后3月的IKDC评分[MD=7.07,95%CI(2.65,11.49),P=0.0002],改善术后2周[MD=-2.09,95%CI(-4.17,-0.00),P=0.05]、4周[MD=-2.54,95%CI(-4.08,-0.99),P=0.0001]的肿胀度,降低术后4周[MD=-0.89,95%CI(-1.70,-0.09),P=0.03]、8周[MD=-0.36,95%CI(-0.67,-0.06),P=0.02]、12周[MD=-0.25,95%CI(-0.35,-0.14),P=0.93]的VAS评分,提高术后4周的关节活动度[MD=10.44,95%CI(1.22,19.66),P=0.03]。结论:现有证据显示中药熏洗疗法应用于前交叉韧带重建术后,能改善Lysholm评分、IKDC评分、肿胀度、VAS评分以及关节活动度,但受纳入研究数量和质量的限制,上述结论尚待更多高质量研究予以验证。Objective To systematically review the efficacy of Chinese medicine fumigation for the treatment of recovery after anterior cruciate ligament reconstruction operation.Methods PubMed,EMbase,The Cochrane Library,Web of Science,CNKI,CBM,WanFang and VIP databases were electronically searched to collect randomized controlled trials(RCTs)and cohort studies about unicompartmental knee arthroplasty on FJS in patients with knee osteoarthritis from inception to November 30st,2020.Two reviewers independently screened literature,extracted data and assessed the risk of bias of included studies,then,meta-analysis was performed by using RevMan 5.3 software.Results A total of 6 RCTs were included.Meta-analysis showed that:Chinese medicine fumigation for the treatment of recovery after anterior cruciate ligament reconstruction operation,could improve Lysholm on 3th month(MD=3.27,95%CI(1.97,4.56),P<0.00001)after operation,improve IKDC on 3th month(MD=7.07,95%CI(2.65,11.49),P=0.0002)after operation,improve degree of swelling on 2th week(MD=-2.09,95%CI(-4.17,-0.00),P=0.05),4th week(MD=-2.54,95%CI(-4.08,-0.99),P=0.0001)after operation,reduce VAS on 4th week(MD=-0.89,95%CI(-1.70,-0.09),P=0.03),8th week(MD=-0.36,95%CI(-0.67,-0.06),P=0.02),12th week(MD=-0.25,95%CI(-0.35,-0.14),P=0.93)after operation,improve range of motion on 4th week(MD=10.44,95%CI(1.22,19.66),P=0.03)after operation.Conclusion Current evidence shows that,Chinese medicine fumigation for the treatment of recovery after anterior cruciate ligament reconstruction operation could improve Lysholm,IKDC,degree of swelling,VAS and range of motion.Due to limited quality and quantity of the included studies,more high quality studies are needed to verify above conclusion.
