不典型与典型肺结核痰抗酸杆菌、糖类抗原125、D-二聚体、腺苷脱氨酶检测结果对比

摘　　要：目的对比不典型与典型肺结核痰抗酸杆菌、糖类抗原(Carbohydrate antigen,CA)125、D-二聚体、腺苷脱氨酶(Adenosine deaminase,ADA)检测结果。方法选择2019年2月~2021年5月我院诊治的肺结核患者118例作为研究对象,根据临床症状将患者分为典型症状组(78例)和不典型症状组(40例),检测两组痰抗酸杆菌阳性情况及血清CA125、D-二聚体、ADA水平并进行相关性分析。结果典型症状组痰抗酸杆菌阳性率为98.7%,高于不典型症状组的85.0%(P<0.05)。典型症状组血清CA125、D-二聚体、ADA水平均高于不典型症状组(P<0.05)。Spearman分析显示,CA125、D-二聚体、ADA、痰抗酸杆菌阳性均与典型肺结核的发生呈正相关(P<0.05)。受试者工作特征(Receiver operating characteristic,ROC)曲线显示,CA125、D-二聚体、ADA、痰抗酸杆菌阳性诊断典型肺结核的曲线下面积(Area under curve,AUC)分别为0.672、0.593、0.713和0.922。结论相对于不典型肺结核,典型肺结核多表现为痰抗酸杆菌阳性及血清CA125、D-二聚体、ADA水平升高,其与典型肺结核的发生存在相关性,也是诊断肺结核的重要指标。Objective To compare the test results of sputum acid-fast bacilli, carbohydrate antigen(CA) 125,D-dimer and adenosine deaminase(ADA) between atypical and typical pulmonary tuberculosis. Methods 118 patients with pulmonary tuberculosis diagnosed and treated in our hospital from Feb 2019 to May 2021 were selected as the research objects. All the patients were divided into typical symptom group of 78 cases and atypical symptom group of 40 cases according to the clinical symptoms. Sputum acid-fast bacillus, the serum levels of CA125, D-dimer and ADA were detected in the two groups at the same time, and the correlation analysis was performed. Results The positive rate of sputum acid-fast bacilli in typical symptom group was 98.7%, which was higher than that(85.0%) in atypical symptom group(P<0.05). The levels of serum CA125, D-dimer, ADA in typical symptom group were higher than those in atypical symptom group(P<0.05). Spearman analysis showed that CA125, D-dimer, ADA and positive sputum acid-fast bacilli were positively correlated with the occurrence of typical pulmonary tuberculosis(P<0.05).The receiver operating characteristic(ROC) curve showed that area under the curve(AUC) of CA125, D-dimer,ADA, positive sputum acid-fast bacilli in the diagnosis of typical pulmonary tuberculosis were 0.672, 0.593, 0.713 and 0.922 respectively. Conclusion Compared with atypical pulmonary tuberculosis, typical pulmonary tuberculosis is mostly characterized by positive sputum acid-fast bacilli and high serum levels of CA125, D-dimer and ADA. They are correlated with the occurrence of typical pulmonary tuberculosis and also important indicators for the diagnosis of pulmonary tuberculosis.
