降香通脉汤对急性ST段抬高心肌梗死患者再灌注后微循环障碍的影响

摘　　要：目的:观察降香通脉汤对急性ST段抬高心肌梗死(STEMI)患者再灌注后微循环障碍的影响。方法:选取STEMI患者100例,采用数字表法随机分为两组各50例。对照组予常规西药治疗,治疗组在对照组的基础上加用降香通脉汤,治疗14天后观察两组临床疗效、生存质量评价、炎症指标水平(RDW、NLR、MHR)、血脂水平(LDL-C和B/A1)及血管内皮功能的变化。结果:治疗组临床疗效优于对照组,差异有统计学意义(P﹤0.01);生存质量评价指标优于对照组,差异有统计学意义(P﹤0.01);炎症指标血脂水平及微循环指标均优于对照组,差异具有统计学意义(P﹤0.01)。结论:对于STMEI患者再灌注后仍有胸闷痛发作的患者,术后常规西药治疗的基础上加用降香通脉汤能改善炎症及血脂水平,改善患者临床症状,提示患者生活质量。Objective:To observe the clinical effect of Jiangxiang Tongmai decoction on reperfusion-caused microcirculation disturbance in patients with acute ST-segment elevation myocardial infarction(STEMI).Methods:100 cases of selected patients with STEMI were randomly divided into two groups,50 cases in each group.The control group was treated with conventional western medicine,while the treatment group was treated plus with Jiangxiang Tongmai decoction on the basis of control group.After 14 days of treatment,the changes of clinical efficacy indexes,life-quality evaluation,inflammatory indexes level(RDW,NLR,MHR),blood lipid level(LDL-C and B/A1)and vascular endothelial function indexes were observed in two groups.Results:The clinical efficacy of treatment group was better than that of control group,and the difference was statistically significant(P﹤0.01).The evaluation indexes of life-quality of treatment group were better than those of control group,and the difference was statistically significant(P﹤0.01).The inflammatory indexes,blood lipid level and microcirculation indexes of treatment group were better than those of control group,and the difference was statistically significant(P﹤0.01).Conclusion:For STEMI patients who have still the attacks of chest stuffy pain after reperfusion,to additionally use Jiangxiang Tongmai decoction on the basis of treatment with routine western medicine after operation can improve the levels of inflammation and blood lipid and the clinical symptoms of patients,and enhance the life-quality of patients.
