健心颗粒治疗气虚血瘀型老年射血分数保留心力衰竭临床观察

摘　　要：目的:观察健心颗粒对气虚血瘀型老年射血分数保留心力衰竭(HFpEF)患者中医证候和心功能疗效及血清C反应蛋白(CRP)、白介素1β(IL-1β)和白介素6(IL-6)水平的影响。方法:将气虚血瘀型老年HFpEF患者150例随机分成两组各75例。对照组按现代医学相关指南共识予规范抗心力衰竭治疗,治疗组在对照组治疗基础上予健心颗粒。8周后,评价两组的中医证候和心功能疗效,观察治疗前后血清N末端B型钠尿肽原(NT-proBNP)、CRP、IL-1β和IL-6的水平。结果:对照组剔除2例,治疗组剔除3例。治疗组中医证候和心功能的总有效率均明显高于对照组(P﹤0.05);治疗后两组NT-proBNP、CRP、IL-1β及IL-6水平均低于治疗前(P﹤0.05),且治疗组各炎症介质浓度明显低于对照组(P﹤0.05)。结论:健心颗粒可以改善气虚血瘀型老年HFpEF患者的中医证候和心功能,降低机体炎症水平。Objective:To observe the clinical effect of Jianxin granule on TCM syndrome,cardiac function,levels of blood serum C-reactive protein(CRP),interleukin-1β(IL-1β)and interleukin-6(IL-6)of elderly patients with heart failure with preserved ejection fraction(HFpEF)of qi deficiency and blood stasis type.Methods:150 cases of elderly patients with HFpEF of qi deficiency and blood stasis type were randomly divided into two groups,75 cases in each group.The control group was given the standard treatment of anti heart failure according to the consensus of relevant modern medical guidelines,while the treatment group took Jianxin granule on the basis of treatment of control group.After 8 weeks,the curative effects of TCM syndromes and cardiac function were evaluated in two groups,and the levels of serum N-terminal pro-B-type natriuretic peptide(NT-proBNP),CRP,IL-1β and IL-6 were observed before and after treatment.Results:2 cases were excluded in the control group,while 3 cases were excluded in the treatment group.The total effective rates of TCM syndrome and cardiac function in treatment group were significantly higher than those in control group(P﹤0.05).After treatment,the levels of NT-proBNP,CRP,IL-1β and IL-6 in two groups were lower than those before treatment(P﹤0.05),and the concentration of each inflammatory mediator in treatment group was significantly lower than that in control group(P﹤0.05).Conclusion:Jianxin granule can improve TCM syndrome and cardiac function of elderly HFpEF patients with qi deficiency and blood stasis syndrome,and reduce the level of inflammation of organisms.
