基于运动康复最佳证据总结的活动方案在慢性稳定期心力衰竭患者中的应用

摘　　要：目的:探讨基于运动康复最佳证据总结的活动方案在慢性稳定期心力衰竭患者中的应用效果。方法:将2018年7月1日~2020年12月31日收治的240例慢性稳定期心力衰竭患者随机分为对照组和观察组各120例,对照组实施常规护理干预,观察组实施基于运动康复最佳证据总结的活动方案护理;比较两组临床指标[包括第一秒用力呼出量占用力肺活量的百分比(FEV_(1)/FVC%)、呼气峰值流速(PEF)、第一秒用力呼吸量(FEV_(1))、最大呼气中段平均流速(MMEF)、6分钟步行试验(6MWD)]、生活质量及睡眠情况[采用瑞典慢性心力衰竭患者睡眠困难(USI-CHF)]。结果:观察组FEV_(1)/FVC%、PEF、FEV_(1)、MMEF及6MWD优于对照组(P<0.01),社会限制、情绪、体力限制及临床症状得分高于对照组(P<0.01);观察组入睡障碍、维护障碍、早醒及睡眠障碍总发生率低于对照组(P<0.05)。结论:对慢性稳定期心力衰竭患者实施基于运动康复最佳证据总结的活动方案,可有效改善患者各项临床指标,提高患者生活质量水平,减轻患者睡眠障碍,对患者临床的效果起到了促进作用。Objective:To explore the application effect of exercise rehabilitation based on best evidence summary in patients with chronic stable heart failure.Methods:A total of 240 patients with chronic stable heart failure admitted during July 1,2019 to December 31,2020 were randomly divided into the control group and the observation group,120 cases each,The control group was given routine nursing intervention,The observation group implemented nursing intervention based on the best evidence summary of exercise rehabilitation,The clinical indexes including FEV_(1)/FVC%,PEF,FEV_(1),MMEF,6MWD,quality of life and sleep USI-CHF in the two groups were compared.Results:The 1 second specific exertional vital capacity(FEV_(1)/FVC),peak expiratory flow rate(PEF),1 second exertional respiration rate(FEV_(1)),maximum expiratory mean flow rate(MMEF)and 6 min walking test(6 MWD)in the observation group were higher than those in the control group(P<0.05);The scores of incidence of sleep disturbance,maintenance disturbance,early awakening and sleep disturbance in the observation group were lower than those in the control group(P<0.05).Conclusion:Nursing intervention based on the best evidence summary of exercise rehabilitation in patients with chronic stable heart failure,can effectively improve the patient′s clinical indicators,improve their quality of life,reduce sleep disorders,so as to promote the clinical effect in patients.
