摘  要:目的探讨化瘀宁坤汤对盆腔炎大鼠炎症反应及Toll样受体4(TLR4)/核转录因子kappaB(NF-κB)/环氧化酶2(COX2)信号通路的影响。方法实验分为假手术组、模型组、化瘀宁坤汤组、TLR4抑制剂组、化瘀宁坤汤+TLR4抑制剂组,每组10只。除假手术组外,其余各组机械损伤子宫内膜组织并混合菌液0.2 mL注入双侧子宫内构建盆腔炎大鼠模型,15 d后治疗。化瘀宁坤汤组、化瘀宁坤汤+TLR4抑制剂组灌肠6.25 mL化瘀宁坤汤;TLR4抑制剂组、化瘀宁坤汤+TLR4抑制剂组腹腔注射1 mg·kg;Tak-242;假手术组和模型组分别腹腔注射和灌肠等体积生理盐水。实验结束后检测各组大鼠子宫系数;苏木素-伊红(HE)检测子宫组织形态;酶联免疫吸附(ELISA)检测血清中炎症因子白介素-1β(IL-1β)、肿瘤坏死因子-α(TNF-α)水平;免疫浊度法检测血清中免疫球蛋白(Ig)G、IgA和IgM水平;蛋白免疫印迹检测子宫组织中TLR4、NF-κB p65、COX2蛋白水平。结果与假手术组相比,模型组大鼠子宫系数,血清中IL-1β、TNF-α水平,子宫粘连扩张、慢性炎症浸润、内膜充血水肿、上皮变性坏死、上皮细胞增生病理评分,子宫组织中TLR4、NF-κB、COX2蛋白水平升高(P<0.05);血清中IgG、IgA水平降低(P<0.05)。与模型组相比,化瘀宁坤汤组、TLR4抑制剂组、化瘀宁坤汤+TLR4抑制剂组大鼠子宫系数,血清中IL-1β、TNF-α水平,子宫粘连扩张、慢性炎症浸润、内膜充血水肿、上皮变性坏死、上皮细胞增生病理评分,子宫组织中TLR4、NF-κB、COX2蛋白水平降低(P<0.05);血清中IgG、IgA水平升高(P<0.05)。分别与化瘀宁坤汤组、TLR4抑制剂组相比,化瘀宁坤汤+TLR4抑制剂组大鼠子宫系数,血清中IL-1β、TNF-α水平,子宫粘连扩张、慢性炎症浸润、内膜充血水肿、上皮变性坏死、上皮细胞增生病理评分,子宫组织中TLR4、COX2蛋白水平降低(P<0.05);血清中IgG、IgA水平升高(P<0.05)。与化瘀宁坤汤�Objective To investigate the effect of Huayu Ningkun Decoction on the inflammatory responses and Toll-like receptor 4 (TLR4)/nuclear transcription factor kappaB (NF-κB)/cyclooxygenase 2 (COX2) signaling pathway in rats with the pelvic inflammatory disease.Methods The rats in the experiment were randomly divided into a sham operation group,a model group,a Huayu Ningkun Decoction group,a TLR4 inhibitor group,and a Huayu Ningkun Decoction + TLR4 inhibitor group,with 10 rats in each group.Except the rats in the sham operation group,the endometrium tissues were mechanically damaged in the rats of other groups,and mixed 0.2 mL bacterial solution was injected into their bilateral uteruses to establish a rat model of the pelvic infl ammatory disease.The rats were treated 15 days later.The Huayu Ningkun Decoction group and the Huayu Ningkun Decoction + TLR4 inhibitor group were given 6.25 mL Huayu Ningkun Decoction by enema,while the TLR4 inhibitor group and the Huayu Ningkun Decoction + TLR4 inhibitor group were intraperitoneally injected with 1 mg·kg;Tak-242,and the sham operation group and the model group were injected intraperitoneally and given by enema with equal volumes of normal saline,respectively.At the end of the experiment,the uterine coefficient of each group was measured,hematoxylin eosin (HE) was used to detect the morphology of uterus,the levels of interleukin-1β (IL-1β) and tumor necrosis factor-α (TNF-α) in serum were detected by enzyme-linked immunosorbent assay (ELISA),the levels of immunoglobulin (Ig) G,IgA and IgM in serum were detected by immunoturbidimetry,and the levels of TLR4,NF-κB p65 and COX2 were detected by Western blot.Results Compared with the sham operation group,the uterus coefficient,the levels of IL-1β and TNF-α in serum,the pathological scores of adhesions and dilations,chronic inflammatory infiltration,endometrial hyperemia and edema,epithelial degeneration and necrosis,and epithelial cell proliferation,and the levels of TLR4,NF-κB and COX-2 proteins in uterus tissues in th
