蒲公英汤联合麦默通微创置管引流术治疗急性哺乳期乳腺炎并脓肿形成临床研究

摘　　要：目的探讨蒲公英汤联合麦默通微创置管引流术治疗急性哺乳期乳腺炎并脓肿形成的临床疗效。方法选择70例急性哺乳期乳腺炎并脓肿形成患者作为研究对象,采用随机数表法分为对照组与观察组,各35例。对照组给予常规治疗及麦默通微创置管引流术治疗,观察组在对照组基础上给予蒲公英汤治疗,2组均持续治疗6周。比较2组治疗6周后的临床疗效,治疗期间抗生素使用时间、治愈时间、回乳率、乳房变形率,治疗前后乳房肿块直径评分、皮肤变化评分、视觉模拟评分(VAS)、血清免疫球蛋白水平及生活质量评分。结果治疗后,观察组总有效率(94.29%,33/35)高于对照组(74.29%,26/35)(P<0.05)。治疗期间,观察组抗生素使用时间及治愈时间均短于对照组(P<0.05)。与治疗前比较,治疗后2组乳房肿块直径评分、皮肤变化评分、VAS评分及血清免疫球蛋白G(IgG)、免疫球蛋白A(IgA)、免疫球蛋白M(IgM)水平均降低,且观察组低于对照组(P<0.05);2组生理功能、情感功能、社交评分均升高,且观察组高于对照组(P<0.05)。结论蒲公英汤联合麦默通微创置管引流术可减轻急性哺乳期乳腺炎并脓肿形成患者疼痛,缩小乳房肿块直径,减少皮肤松弛,降低患者回乳率及乳房变形率,促进患者病情恢复,缩短抗生素使用时间及缩短治愈时间,提高生活质量,疗效显著。Objective To explore the clinical efficacy of Pugongying Decoction combined with the Mammotome minimally invasive catheter drainage in the treatment of acute lactation mastitis with the abscess formation.Methods A total of 70 cases of acute lactation mastitis with an abscess formation were selected as the research subjects and were divided into a control group and an observation group by the random number table method,with 35 cases in each group.The control group was given a conventional treatment and the Mammotome minimally invasive catheter drainage,while the observation group was given Pugongying Decoction on the basis of the control group,both for 6 weeks.The clinical efficacy,antibiotics use time,cure time,breast reversion rate,breast deformation rate,breast mass diameter score,skin change score,visual analog scale (VAS) score,serum immunoglobulin level and quality of life score were compared between the two groups after 6 weeks of treatment.Results After treatment,the total effective rate of the observation group (94.29%,33/35) was higher than that of the control group (74.29%,26/35) (P<0.05).During the treatment,the antibiotics use time and cure time in the observation group were shorter than those in the control group (P<0.05).Compared with before treatment,the breast mass diameter score,skin change score,VAS score and serum immunoglobulin G (IgG),immunoglobulin A (IgA),immunoglobulin M (IgM) levels in both groups were decreased after treatment,and the observation group was lower than the control group (P<0.05).The physiological function,emotional function and social score of the two groups were increased,and the observation group was higher than the control group (P<0.05).Conclusion Pugongying Decoction combined the Mammotome minimally invasive drainage can reduce pain in patients with acute lactation mastitis with the abscess formation,reduce the diameter of breast lumps,skin sagging,the rate of breast reversion and breast deformation and promote the recovery of patients,shorten the antibiotics use time
