摘 要:目的探讨标准大骨瓣减压术治疗颅脑外伤的临床效果。方法 52例颅脑外伤患者,根据随机数字表法分为标准大骨瓣组和传统对照组,各26例。传统对照组采用传统规骨瓣开颅手术治疗,标准大骨瓣组采用标准大骨瓣减压术治疗。对比两组治疗前后格拉斯哥预后评分、生存质量评分、神经功能评分,治疗效果,并发症发生情况和住院时间。结果治疗后,标准大骨瓣组格拉斯哥预后评分、生存质量评分、神经功能评分均优于传统对照组,差异均有统计学意义(P<0.05)。标准大骨瓣组治疗总有效率92.31%高于传统对照组的65.38%,并发症发生率3.85%低于传统对照组的26.92%,差异有统计学意义(P<0.05)。标准大骨瓣组住院时间(11.34±2.01)d短于传统对照组的(14.67±2.45)d,差异有统计学意义(P<0.05)。结论标准大骨瓣减压术治疗颅脑外伤效果确切,值得临床推广使用。Objective To discuss the clinical effect of standard large bone flap decompression in the treatment of craniocerebral trauma. Methods A total of 52 patients with craniocerebral trauma were randomly divided into standard large bone flap group and traditional control group according to the random numerical table,with 26 cases in each group. The traditional control group was treated with traditional craniotomy, and the standard large bone flap group was treated with standard large bone flap decompression. Both groups were compared int terms of Glasgow prognosis score, quality of life score and neurological function score before and after treatment,therapeutic effect, complication rate and hospitalization time. Results After treatment, the Glasgow prognosis score, quality of life score, and neurological function score of the standard large bone flap group were better than those of the traditional control group, and the differences were statistically significant(P<0.05). The total effective rate of treatment 92.31% in the standard large bone flap group was higher than 65.38% in the traditional control group, and the complication rate 3.85% was lower than 26.92% in the traditional control group, and the difference was statistically significant(P<0.05). The hospitalization time(11.34±2.01) d in the standard large bone flap group was shorter than(14.67±2.45) d in the traditional control group, and the difference was statistically significant(P<0.05). Conclusion Standard large bone flap decompression has a definite effect in the treatment of craniocerebral trauma, and is worthy of clinical promotion and application.
