摘  要:目的探讨呼出气一氧化氮(FeNO)、免疫球蛋白E(IgE)、嗜酸粒细胞阳离子蛋白(ECP)联合检测对学龄期支气管哮喘患儿的诊断价值。方法选择2020年1—9月河南省某医院收治的100例学龄期支气管哮喘患儿为观察组,选择同期在该院进行体检的100名健康儿童作为对照组。比较2组研究对象的FeNO、IgE、ECP水平和最大呼气流量占预计值百分比(PEF%)、第1秒用力呼气容积占预计值百分比(FEV1%)。并绘制受试者工作曲线(ROC),分析FeNO、IgE、ECP联合诊断学龄期支气管哮喘的效能。结果观察组研究对象FeNO、IgE、ECP水平高于对照组,差异均有统计学意义(P<0.05)。观察组研究对象PEF%、FEV1%低于对照组,差异均有统计学意义(P<0.05)。FeNO、IgE、ECP水平分别与PEF%、FEV1%呈负相关(P<0.05)。FeNO、IgE、ECP联合诊断学龄期支气管哮喘的曲线下面积(AUC)为0.891,特异度、敏感度分别为0.974、0.785,均高于各指标单一检测的数值。结论FeNO、IgE、ECP水平升高与学龄期支气管哮喘的发生密切相关,与患者肺功能相关指标呈负相关,三者联合检测学龄期支气管哮喘具有较高敏感度和特异度,能够提高检出率,值得临床推广。Objective To investigate the diagnostic value of combined fractional exhaled nitric oxide(FeNO),immunoglobulin E(IgE),and eosinophil cationic protein(ECP)tests on school-age children with bronchial asthma.Methods One hundred cases of school-age bronchial asthma children admitted to a hospital in Henan province from January to September 2020 were selected as the observation group,and 100 healthy children who underwent physical examination at the hospital during the same period were selected as the control group.The FeNO,IgE,ECP levels and peak expiratory flow as a percentage of the predicted value(PEF%)and the 1st second forced expiratory volume as a percentage of the predicted value(FEV1%)were compared between the 2 groups.The subjects′receiver operating characteristic(ROC)curved were also plotted to analyze the efficacy of FeNO,IgE,and ECP in the combined diagnosis of bronchial asthma in school age.Results The levels of FeNO,IgE,and ECP were higher in the study subjects of the observation group than in the control group,and the differences were all statistically significant(P<0.05).The PEF%and FEV1%of the study subjects in the observation group were lower than those in the control group,and the differences were all statistically significant(P<0.05).The levels of FeNO,IgE,and ECP were negatively correlated with PEF%and FEV1%,respectively(P<0.05).The area under the curve(AUC)of FeNO,IgE,and ECP combined to diagnose bronchial asthma in school age was 0.891,and the specificity and sensitivity was 0.974 and 0.785,respectively,which were higher than the values of single test for each index.Conclusion Elevated levels of FeNO,IgE,and ECP are closely related to the occurrence of bronchial asthma in school-aged patients,and negatively correlated with patients′lung function indexes.The combination of the three tests has high sensitivity and specificity in detecting bronchial asthma in school-aged patients,which can improve the detection rate and thus provide reference for treatment,and therefore,it is worthy of clinica
