腔内非离断式Roux-en-Y吻合术在全腹腔镜远端胃癌根治术消化道重建中的应用

摘　　要：目的针对全腹腔镜远端胃癌根治术患者采用腔内非离断式Roux-en-Y吻合术,对其效果进行总结评价。方法选取2019年11月至2020年11月江西省肿瘤医院80例全腹腔镜远端胃癌根治术患者作为研究对象,按照随机数字表法分为观察组(腔内非离断式Roux-en-Y吻合术)与对照组(传统Roux-en-Y吻合术)两组,每组各40例。记录两组手术情况及恢复情况,随访并发症类型及占比情况。结果观察组患者手术时间、消化道重建时间及吻合时间短于对照组,术中出血量少于对照组,差异有统计学意义(P<0.05);观察组患者术后排气时间、进食流质、半流质时间及住院时间短于对照组,差异有统计学意义(P<0.05);两组并发症发生率比较,观察组低于对照组,差异有统计学意义(P<0.05)。结论全腹腔镜远端胃癌根治术患者在消化道重建中选择腔内非离断式Roux-en-Y吻合术,手术创伤小、术后恢复快,并发症较传统Roux-en-Y吻合少,可予以推广。Objective To summarize and evaluate the effect of intracavitary uncut Roux-en-Y anastomosis for patients undergoing totally laparoscopic distal gastrectomy.Methods A total of 80 patients undergoing totally laparoscopic distal gastrectomy in Jiangxi Cancer Hospital from November 2019 to November 2020 were selected as study subjects.They were divided into the observation group(intracavitary uncut Roux-en-Y anastomosis)and the control group(traditional Roux-en-Y anastomosis)according to the random number table method,with 40 cases in each group.The surgery and recovery of both groups were recorded,and the type and percentage of complications were followed up.Results The operating time,time for gastrointestinal tract reconstruction and anastomosis time of patients in the observation group were shorter than those in the control group,and the intraoperative bleeding volume was less than that in the control group,with statistically significant differences(P<0.05).The time of postoperative exhaustion,the time of eating liquid and semi-liquid food and the hospital stay of patients in the observation group were shorter than those in the control group,with statistically significant differences(P<0.05).The incidence of complications in the observation group was lower than that in the control group,with statistically significant differences(P<0.05).Conclusion In the gastrointestinal tract reconstruction of patients undergoing totally laparoscopic distal gastrectomy,intracavitary uncut Roux-en-Y anastomosis is less invasive,with faster postoperative recovery and fewer complications than traditional Roux-en-Y anastomosis,and can be promoted.
