基于“阳虚阴结”理论应用温阳化瘀法治疗糖尿病足溃疡

摘　　要：本文依据《内经》"阳化气,阴成形"理论,提出糖尿病足溃疡的病机关键在于"阳虚阴结"。阳气亏虚,肌肤失于温煦,气血化生、运行无力,阴寒内结,聚而成瘀,化为有形之邪,阻碍新肉生长,导致糖尿病足溃疡。法当以温阳散寒治本、活血化瘀治标,采用温阳与化瘀相结合原则,随证选用补肾健脾、活血祛瘀之品温阳化瘀,标本兼顾,内服以肾气丸合理中汤、桃红四物汤为主,外治以温阳生肌膏。According to the theory of“Yang transforming qi and yin forming shape”in Nei Jing,the key to pathogenesis of diabetic foot ulcer is“yang deficiency and yin stagnation”.Deficiency of yang qi,skin loss in warm,qi and blood failing in transforming and promoting,yin and cold in the knot,gathering into blood stasis,into visible evil,and hindering the growth of new flesh are factors leading to the formation of diabetic foot ulcer.In the treatment,warming yang and dispersing cold is the basic treatment,and promoting blood circulation and removing stasis is the temporary treatment.The treatment principle of warming yang and removing blood stasis is adopted.According to the syndrome,the medicine of tonifying kidney and spleen,promoting blood circulation and removing blood stasis are selected.The roots and symptoms of warming yang and removing blood stasis are taken into account.The main treatment is Shenqi Pills,combined with Lizhong Decoction and Taohong Siwu Decoction.The external treatment is Wenyang Shengji Ointment.
