摘 要:目的表面活性物质治疗实验兔肺挫伤效果的CT影像学评估研究。方法15只健康成年雄性新西兰兔根据实验方案分为对照组(正常兔,不进行干预),肺挫伤组(重物撞击诱导急性肺损伤模型兔,不进行干预),治疗组(在诱发急性肺挫伤后30min,气管内给予10mg/kg剂量的猪肺磷脂表面活性剂干预),各5只。使用ABL5血气分析仪检测兔氧分压(PO_(2))和血氧分压(PaO_(2))。通过市售酶联免疫吸附试验(ELISA)试剂盒检测兔血浆白细胞介素(IL)-1β、肿瘤坏死因子α(TNF-α)、IL-10和IL-13水平。使用免疫组织病理学评分系统比较兔肺挫伤程度。通过纤维支气管镜回收兔支气管肺泡灌洗(BAL)液并对中性粒细胞、SLPI和白蛋白含量进行检测。通过CT扫描和3D-Doctor半自动图像分析比较各组兔的肺通气情况。结果肺挫伤后1h,肺挫伤组较对照组PO_(2)显著降低,PaO_(2)显著升高,差异均有统计学意义(P<0.05),而治疗组和肺挫伤组PO_(2)和PaO_(2)比较差异无统计学意义(P>0.05);肺挫伤后2~4h,肺挫伤组较对照组PO_(2)显著降低,PaO_(2)显著升高,而治疗组较肺挫伤组PO_(2)显著升高,PaO_(2)显著降低,差异均有统计学意义(P<0.05)。肺挫伤组较对照组IL-1β和TNF-α水平升高,IL-10和IL-13水平降低,差异均有统计学意义(P<0.05);治疗组较肺挫伤组IL-1β和TNF-α水平降低,IL-10和IL-13水平升高,差异均有统计学意义(P<0.05)。肺挫伤组较对照组的肺泡出血、肺泡水肿/充血和白细胞浸润评分升高,差异均有统计学意义(P<0.05);治疗组较肺挫伤组的肺泡出血、肺泡水肿/充血和白细胞浸润评分降低,差异均有统计学意义(P<0.05)。肺挫伤组较对照组中性粒细胞百分比、白细胞中性蛋白酶抑制剂(SLPI)和白蛋白浓度升高,差异均有统计学意义(P<0.05);治疗组较肺挫伤组中性粒细胞百分比和白蛋白浓度降低,差异均有统计学意义(P<0.05)。肺挫伤组较对照组的全肺灰度Objective Research on CT imaging evaluation of surfactant treatment of experimental rabbit pulling effect.Methods Fifteen healthy adult male New Zealand rabbits were divided into control group(normal rabbits without intervention),lung contusion group(a rabbit model of acute lung injury induced by heavy object impact without intervention)and the treatment group(30 min after induction of acute lung contusion,intratracheal administration of 10 mg/kg dose poractant olfa surfactant intervention),5 rats in each group.ABL5 blood gas analyzer was used to detect rabbit PO_(2)and PaO_(2).The levels of rabbit plasma IL-1β,TNF-α,IL-10 and IL-13 were detected by commercially available enzyme-linked immunosorbent assay(ELISA)kits.The immunohistopathology scoring system was used to compare the degree of rabbit lung contusion.The rabbit bronchoalveolar lavage(BAL)fluid was recovered by fiberoptic bronchoscopy and the neutrophil,SLPI and albumin content were detected.The lung ventilation of each group of rabbits was compared by CT scan and 3D-Doctor semi-automatic image analysis.Results In the first hour after lung contusion,the PO_(2)of the lung contusion group was significantly lower than that of the control group,PaO_(2)was significantly higher than that of the control group,the differences were statistically significant(P<0.05),but there was no difference in PO_(2)and PaO_(2)between the treatment group and the lung contusion group(P>0.05);2 to 4 hours after lung contusion,the PO_(2)of the lung contusion group was significantly lower than that of the control group,PaO_(2)was significantly higher than that of the control group,and the PO_(2)of the treatment group was significantly higher than that of the lung contusion group,PaO_(2)was significantly lower than that of the lung contusion group,the differences were statistically significant(P<0.05).Compared with the control group,the levels of IL-1βand TNF-αwere higher in the lung contusion group,and the levels of IL-10 and IL-13 were lower,the differences were statistically
