摘  要:目的研究骨关节病变诊断中磁共振脂肪抑制技术的应用价值。方法采用回顾性分析方法,以2019年9月至2021年5月中国中医科学院广安门医院收治入院的120例疑似骨关节病变患者为研究对象,所有患者均接受X线、磁共振成像(MRI)检查;比较MRI T1WI、MRI T2WI、MRI脂肪抑制、X线诊断骨髓病变(骨质、骨膜皮质、骨关节骨髓、骨损伤)的敏感度,并比较MRI T1WI、MRI T2WI、MRI脂肪抑制诊断骨关节病变伴随损伤(积液、韧带、肌腱、微骨折)阳性率及成像质量(直接辨认、容易辨认、辨认难度一般、辨认难度高)。结果 MRI T1WI、MRI T2WI、X线诊断骨质、骨膜皮质、骨损伤敏感度差异无统计学意义(P> 0.05);与X线比较,MRI T1WI、MRI T2WI诊断骨关节骨髓敏感度更高,差异均有统计学意义(P <0.05);MRI T1WI、MRI T2WI诊断骨关节骨髓敏感度差异无统计学意义(P> 0.05);与MRI T1WI、MRI T2WI、X线比较,MRI脂肪抑制诊断骨质、骨膜皮质、骨关节骨髓、骨损伤的敏感度更高,差异均有统计学意义(P <0.05)。MRI T1WI、MRI T2WI、MRI脂肪抑制诊断积液、肌腱、微骨折的阳性率差异无统计学意义(P>0.05);与MRI T1WI、MRI T2WI比较,MRI脂肪抑制诊断韧带损伤的阳性率更高,差异有统计学意义(P <0.05)。与MRI T2WI比较,MRI T1WI的骨髓成像质量更高,差异有统计学意义(P <0.05);与MRI T1WI、MRI T2WI比较,MRI脂肪抑制技术的骨髓成像质量更高,差异有统计学意义(P <0.05)。结论骨关节病变诊断中MRI脂肪抑制技术的应用效果显著,骨关节病变诊断准确性高,可清晰显示并观察骨髓病变及伴随病变。Objective To study the application value of magnetic resonance fat suppression technique in the diagnosis of osteoarthropathy.Methods The retrospective analysis method was adopted. From September 2019 to May 2021,120 patients with suspected bone and joint disease admitted to Guang’anmen Hospital,China Academy of Chinese Medical Sciences were selected as the research objects. All patients underwent X-ray and magnetic resonance imaging( MRI) examinations. The sensitivity of MRI T1WI,MRI T2WI,MRI fat suppression,and X-ray diagnosis of bone marrow lesions( bone,periosteum cortex,bone joint bone marrow,and bone injury) were compared. The positive rate of MRI T1WI,MRI T2WI and MRI fat suppression in the diagnosis of osteoarthropathy accompanied by injury( effusion,ligament,tendon,microfracture) and imaging quality( direct recognition,easy recognition,general recognition,high recognition difficulty) were compared. Results There was no significant difference in the sensitivity of MRI T1WI,MRI T2WI and X-ray diagnosis of bone,periosteum cortex and bone injury( P > 0. 05). Compared with X-ray,MRI T1WI and MRI T2WI showed higher sensitivity in diagnosis of bone,joint and bone marrow,the differences were statistically significant( P < 0. 05). There was no significant difference in sensitivity between MRI T1WI and MRI T2WI in diagnosis of bone joint and bone marrow( P > 0. 05). Compared with MRI T1WI,MRI T2WI,X-ray,MRI adipose suppression had higher sensitivity in diagnosing bone,periosteum cortex,bone joint,bone marrow and bone injury,the differences were statistically significant( P < 0. 05). There was no significant difference in the positive rate of MRI T1WI,MRI T2WI and MRI fat suppression in the diagnosis of effusion,tendon and microfracture( P > 0. 05).Compared with MRI T1WI and T2WI,MRI fat inhibition had a higher positive rate in the diagnosis of ligament injury,the difference was statistically significant( P < 0. 05).
