慢性鼻-鼻窦炎伴鼻息肉的微小RNA表达及T淋巴细胞亚群分布情况研究

摘　　要：目的探讨慢性鼻-鼻窦炎伴鼻息肉(chronic rhino-sinusitis with nasal polyps,CRSwNP)患者的微小RNA(micro RNA,miRNA)-125b、miRNA-34a表达与T淋巴细胞亚群分布情况。方法收集2019年5月至2021年1月于黑龙江省医院就诊的慢性鼻-鼻窦炎患者99例,根据是否伴有鼻息肉分为CRSwNP组(52例)与慢性鼻-鼻窦炎不伴鼻息肉(chronic rhinosinusitis without nasal polyps,CRSsNP)组(47例),选取同期健康体检者(无慢性鼻-鼻窦炎)50例为对照组,比较各组鼻黏膜组织中miRNA-125b、miRNA-34a的表达水平及外周血嗜酸性粒细胞阳性率和T淋巴细胞亚群的分布情况,并进行相关性分析。结果CRSwNP组嗜酸性粒细胞阳性率为92.31%,高于CRSsNP组(74.47%)及对照组(8.00%),三组比较差异有显著性(χ^(2)=7.335,P<0.05)。CRSwNP组的CD^(+)_(3)、CD^(+)_(4)、CD^(+)_(8)T细胞水平均低于对照组和CRSsNP组,CRSwNP组的CD_(4)/CD_(8)比值高于对照组和CRSsNP组,差异有显著性(P<0.05)。对照组鼻黏膜组织中miRNA-125b和miRNA-34a表达水平低于CRSwNP组、CRSsNP组,且CRSsNP组低于CRSwNP组,差异均有显著性(P<0.05)。miRNA-125b、miRNA-34a相对表达水平与嗜酸性粒细胞阳性率、CD^(+)_(3)、CD^(+)_(4)、CD^(+)_(8)T细胞水平及CD_(4)/CD_(8)比值均呈明显正相关(P<0.05)。结论miRNA-125b、miRNA-34a相对表达水平及T淋巴细胞亚群分布变化与慢性鼻-鼻窦炎患者鼻息肉的发生密切相关,对CRSwNP的靶向治疗及预后评估具有一定指导意义。s:Objective Expression of miRNA-125b,miRNA-34a and T lymphocyte subsets in chronic rhinosinusitis with polyps.Method A total of 99 patients with chronic rhinosinusitis who attended in Heilongjiang Provincial Hospital from May 2019 to January 2021 were collected.CRSwNP patients and CRSsNP the distribution of miRNA-125b,miRNA-34a,eosinophils and T lymphocyte subsets in polyp tissues and nasal mucosa tissues of patients,and conduct correlation studies.Result The positive rate of eosinophils in group CRSwNP was 92.31%,higher than that of group CRSsNP(74.47%)and control group(8.00%),and it was significant(χ^(2)=7.335,P<0.05).CD^(+)_(3),CD^(+)_(4),and CD^(+)_(8)levels were lower than in the control and CRSsNP groups,and CD_(4)/CD_(8) was higher in the CRSwNP and CRSsNP groups,and the difference was significant(P<0.05).The expression levels of miRNA-125b and miRNA-34a in the control nasal mucosa were lower than in the CRSwNP and CRSsNP groups(P<0.05),and the miRNA-125b and miRNA-34a were lower than in the CRSwNP group(P<0.05).The relative expression levels of miRNA-125b,and miRNA-34a were significantly associated with eosinophil levels,CD^(+)_(3),CD^(+)_(4),CD^(+)_(8),and CD_(4)/CD_(8) ratios(P<0.05).Conclusion The relative expression levels of miRNA-125b and miRNA-34a are closely related to T lymphocyte subsets in the pathogenesis of nasal polyps,and are of great significance for CRSwNP targeted therapy and prognosis.
