睑结膜入路去睑袋眶隔脂肪纳米化回植对泪沟畸形及黑眼圈的疗效研究

摘　　要：目的:探讨睑结膜入路去睑袋眶隔脂肪纳米化回植对泪沟畸形及黑眼圈的临床疗效。方法:对2018-2020年笔者医院的25例眼袋突出伴轻、中度泪沟畸形及黑眼圈的患者,对其经睑结膜入路去睑袋,适量切除眶隔脂肪,将切除的眶隔脂肪用纳米脂肪转换器制成纳米脂肪,并将其用27G注脂针回植于泪沟和黑眼圈区域,观察其临床效果。结果:对所有患者进行随访,术后6个月,患者泪沟畸形等级量表评分与术前相比降低,其差异具有统计学意义(P<0.05),患者黑眼圈区域的L*值增大,其差异具有统计学意义(P<0.05),a*值、b*值无明显变化,其差异无统计学意义(P>0.05),有2例患者术后出现皮肤瘀斑、1例出现结膜充血,所有患者均无严重并发症出现,患者满意率为92%。结论:睑结膜入路去睑袋眶隔脂肪纳米化回植可有效治疗无明显皮肤松弛的眼袋、轻中度泪沟畸形及黑眼圈的年轻患者。Objective To explore the clinical effect of eyelid bag orbital septum fat nanocrystalization reimplantation by transconjunctival lower eyelid blepharoplasty on tear groove deformity and dark circles.Methods From 2018 to 2020,in our hospital 25 patients with prominent eye bags accompanied by mild to moderate tear groove deformities and dark circles were treated with transconjunctival approach to remove the eyelid pouch,and the orbital septum fat was removed appropriately.Then,the orbital septum fat was made into nanofat by nanofat converters,and it was implanted back into lacrimal groove and dark circles area with 27G fat injection needle.Observing its clinical effect.Results All patients were followed up.At 6 months after operation,the tear trough rating scale of the patients was lower than that before the operation,and the difference was statistically significant(P<0.05).The L*value of the dark circles area of the patients increase,the difference was statistically significant(P<0.05),there was no significant change in a*value and b*value,and the difference was not statistically significant(P>0.05).2 patients had skin ecchymosis,1 patient had conjunctival hyperemia,all patients had no serious complications,and the patient satisfaction rate was 92%.Conclusion Eyelid bag orbital septum fat nanocrystalization reimplantation by transconjunctival lower eyelid blepharoplasty can effectively treat young patients with no obvious skin sagging eye bags,mild to moderate tear groove deformities and dark circles.
