IPL联合595 nm脉冲染料激光治疗玫瑰痤疮的疗效及安全性分析

摘　　要：目的:探究强脉冲光(Intense pulsed light,IPL)联合595 nm脉冲染料激光(PDL)治疗玫瑰痤疮的疗效和安全性。方法:选取2019年1月-2020年1月笔者科室就诊的80例玫瑰痤疮患者进行回顾性分析,其中仅采用595 nm PDL治疗的38例患者为对照组,采用IPL联合595 nm PDL治疗的42例患者为观察组。比较两组治疗效果并分析其安全性。结果:观察组治疗总有效率显著高于对照组,差异有统计学意义(P<0.05);治疗后,观察组红斑毛细血管扩张、脓疱丘疹、瘙痒、红斑评分及DLQI生活质量评分均显著低于对照组,差异有统计学意义(P<0.05);治疗后,观察组C反应蛋白(CRP)、肿瘤坏死因子(TNF-α)、白介素-6(IL-6)水平均显著低于对照组,差异有统计学意义(P<0.05);观察组在治疗方式、治疗效果、术后护理方面满意度均显著高于对照组,差异有统计学意义(P<0.05);治疗后两组均未出现瘢痕、色素沉着、结痂及水疱等不良反应,且肝肾功能及眼底检查未见异常。结论:IPL联合595 nm PDL治疗玫瑰痤疮疗效明显,可有效改善红斑、血管扩张症状,患者满意度高,降低对其生活质量的影响,且安全性较好。Objective To explore the efficacy and safety of intense pulsed light(IPL)combined with 595 nm pulsed dye laser(PDL)in the treatment of acne rosacea.Methods The study retrospectively reviewed 80 patients with acne rosacea treated between January 2019 and January 2020.Among them,38 patients only treated with 595 nm PDL were included in control group,while 42 patients treated with IPL and 595 nm PDL were included in observation group.The efficacy and safety were compared between the two groups.Results The total response rate in observation group was significantly higher than that in control group(P<0.05).After treatment,the scores for erythema telangiectasia,pustular papules,pruritus and erythema,and Dermatology Life Quality Index(DLQI)scores of the observation group were significantly lower than those of the control group(P<0.05).The levels of C-reactive protein(CRP),tumor necrosis factor(TNF-α)and interleukin-6(IL-6)in the observation group were significantly lower than those in the control group(P<0.05).The observation group had significantly higher satisfaction than the control group in terms of treatment method,treatment effect,and postoperative care(P<0.05).No adverse reactions such as scars,pigmentation,scabs,and blisters were observed in the two groups.Liver and kidney function,and fundus examinations showed no abnormalities.Conclusion IPL combined with 595 nm PDL is effective in the treatment of acne rosacea.It can effectively relieve erythema and vasodilation,and reduce the impact on quality of life,with high patient satisfaction and good safety.
