摘  要:目的:探讨高频超声诊断新生儿先天性十二指肠梗阻(CDO)的临床价值。方法:回顾性分析2017年1月—2020年12月在福建省妇幼保健院手术确诊为先天性十二指肠梗阻42例患儿的临床资料,对比高频超声与消化道造影对CDO及其病因的诊断符合率。结果:高频超声准确诊断CDO病因37例,其中先天性肠旋转不良20例、环状胰腺10例、十二指肠狭窄或闭锁7例;消化道造影准确诊断CDO病因23例,其中先天性肠旋转不良19例、环状胰腺0例、十二指肠狭窄或闭锁4例。高频超声与消化道造影诊断CDO的病因总符合率分别为80.43%、50.00%,两种方法差异有统计学意义(χ^(2)=9.392,P=0.002)。结论:高频超声对CDO的诊断具有重要的临床价值,可作为首选的术前影像学检查方法。Objective:To investigate the clinical value of High-frequency ultrasound in the diagnosis of congenital duodenal obstruction(CDO) with neonatal. Methods:The clinical data of 42 children diagnosed with congenital duodenal obstruction during 2017 to 2020 in our hospital were analyzed, the diagnostic rate of CDO was compared between High-frequency ultrasound and gastrointestinal imaging. Results:37 cases of CDO were accurately diagnosed by high-frequency ultrasound, including 20 cases of congenital malrotation, 10 cases of annular pancreas, 7 cases of duodenal stenosis or atresia. In 23 cases of CDO, 19 cases of congenital malrotation, 0 cases of annular pancreas, 4 cases of duodenal stricture or atresia were accurately diagnosed by gastrointestinal angiography. The total coincidence rates of high-frequency ultrasound and gastrointestinal angiography in diagnosing CDO were 80.43% and 50.00%, respectively. The difference between the two methods was statistically significant(χ^(2)=9.392, P =0.002). Conclusion:High-frequency ultrasound has important clinical value in the diagnosis of CDO,It can be used as the preferred preoperative imaging examination method.
