经阴道单孔腹腔镜下骶棘韧带悬吊术治疗盆腔器官脱垂效果分析

摘　　要：目的探讨分析盆腔器官脱垂患者行经阴道单孔腹腔镜下紙棘韧带悬吊术治疗的临床效果。方法选取70例盆腔器官脱垂患者,根据治疗方式不同分为对照组与研究组,每组35例。对照组行经阴道紙棘韧带固定术治疗,研究组行经阴道单孔腹腔镜下紙棘韧带悬吊术治疗。对比两组患者围术期指标以及治疗前后盆腔器官脱垂定量分期法(POP-Q)中相关指标改善情况。结果研究组手术时间、术中出血量、术后尿管留置时间、术后排气时间以及住院时间分别为(143.29±7.65)min、(45.20±2.09)ml、(2.30±0.19)d、(1.55±0.22)d、(5.85±0.07)d,对照组分别为(70.02±3.34)min、(69.02±3.21)ml、(3.70±0.23)d、(1.87±0.34)d、(6.34±0.10)d.研究组手术时间长于对照组,术中出血量少于对照组,术后尿管留置时间、术后排气时间以及住院时间均明显短于对照组,差异有统计学意义(PV0.05)。术前,两组POP-Q各指示点测量水平对比,差异无统计学意义(P>0.05);术后6个月,两组POP-Q各指示点测量水平均大幅度提高,且研究组阴道长度(TVL)值高于对照组,差异有统计学意义(P<0.05),但两组其余指示点测量水平对比,差异无统计学意义(P>0.05)。结论盆腔器官脱垂患者行经阴道单孔腹腔镜下紙棘韧带悬吊术治疗的围术期相关指标显著优于经阴道紙棘韧带固定术治疗,并可有效改善POPQ相关指标,临床应用价值高。Objective To analyze the clinical effect of transvaginal single-port laparoscopic sacrospinous ligament suspension in patients with pelvic organ prolapse.Methods 70 Seventy patients with pelvic organ prolapse were selected and divided into control group and study group according to different treatment methods,with 35 cases in each group.The control group was treated with transvaginal sacrospinous ligament fixation,and the study group was treated with transvaginal single-port laparoscopic sacrospinous ligament suspension.Perioperative indicators and the improvement of related indicators in quantitative staging of pelvic organ prolapse(POP-Q)before and after treatment were compared between the two groups.Results The operative time,intraoperative blood loss,postoperative catheter indwell time,postoperative exhaust time and hospital stay in the study group were(143.29±7.65)min,(45.20±2.09)ml,(2.30±0.19)d,(1.55±0.22)d,(5.85±0.07)d,respectively.Control group were(70.02±3.34)min,(69.02±3.21)ml,(3.70±0.23)d,(1.87±0.34)d,(6.34±0.10)d,respectively.The operative time of the study group was longer than that of the control group,the amount of intraoperative blood loss was less than that of the control group,and the postoperative catheter indwelling time,postoperative exhaust time and hospital stay were significantly shorter than that of the control group,with statistical significance(P<0.05).Before surgery,there was no statistical significance in the measurement levels of POP-Q at each indicator point between the two groups(P>0.05).Six months after surgery,the measurement levels of POP-Q at all indicator points were significantly increased in both groups,and the vaginal length(TVL)value in the study group was higher than that in the control group,with statistical significance(P<0.05),but there was no statistical significance in the measurement levels of other indicator points between the two groups(P>0.05).Conclusion In patients with pelvic organ prolapse,transvaginal single-port laparoscopic sacrospinous ligament s
