阴道镜联合HPV检测对提高宫颈病变筛查准确率的影响

摘　　要：目的探讨应用阴道镜联合人乳头状瘤病毒(HPV)检测对于宫颈病变筛查准确率的影响。方法选取200例接受宫颈病变筛查妇女,均进行HPV检测联合阴道镜检查,对于检查异常情况进一步做宫颈活检病理组织检查,将病理检查结果作为诊断金标准,分析采取阴道镜联合HPV检测对于疾病筛查准确率的价值。结果经病理组织检查,200例受检者中筛查出阳性36例(18.00%),其中宫颈上皮内瘤变(CIN)I 24例(12.00%)、CINII 6例(3.00%)、CINIH 4例(2.00%)、宫颈癌2例(1.00%),在阳性检出情况上,HPV检测阳性13.00%(26/200)、阴道镜检查阳性13.50%(27/200)、联合检查阳性17.00%(34/200),联合检查阳性同病理一致性高。联合检查对宫颈病变的诊断敏感性、特异性及准确性均明显高于HPV检测与阴道镜检查,差异有统计学意义(PV0.05)。结论应用阴道镜联合HPV检测可显著提高临床筛查宫颈病变的准确率,值得临床推广与应用。Objective To explore the effect of colposcopy combined with human papillomavirus(HPV)detection on the accuracy of screening for cervical lesions.Methods 200 women who underwent cervical lesions screening were selected for HPV detection combined with colpoposcopy.For abnormal cases,further cervical biopsy and pathological examination were performed.The pathological examination results were taken as the gold diagnostic standard to analyze the value of colpoposcopy combined with HPV detection for the accuracy of disease screening.Results Through pathological examination,36 cases(18.00%)were positive,including 24 cases of cervical intraepithelial neoplasia(CIN)I(12.00%),6 cases of CIN I(3.00%),4 cases of CIN I(2.00%),2 cases of cervical cancer(1.00%).The positive HPV detection was 13.00%(26/200),the positive colposcopy examination was 13.50%(27/200),and the positive combined examination was 17.00%(34/200).The positive combined examination was highly consistent with the pathological examination.The sensitivity,specificity and accuracy of the combined examination in the diagnosis of cervical lesions were significantly higher than those of HPV test and colposcopy,and the difference was statistically significant(P<0.05).Conclusion Colposcopy combined with HPV detection can significantly improve the clinical screening accuracy of cervical lesions,worthy of clinical promotion and application.
