摘　　要：目的:分析医疗失效模式与效应分析(HFMEA)在手术室常规病理标本管理中的应用效果。方法:成立HFMEA工作小组,确定主题,绘制工作流程图,确定手术室常规病理标本管理失效模式,分析确认根本原因,拟定行动计划并组织实施。结果:标本管理各相关质量指标(标本确认规范执行率、术中标本暂存正确率、标本登记准确率、标本送检质量合格率)明显改善(P<0.001)。结论:应用HFMEA对手术室常规病理标本进行管理,可以有效保障手术室常规病理标本的安全,提高手术室护理质量。Objective:To analyze the application effect of Healthcare Failure Mode and Effect Analysis(HFMEA)in the management of routine pathological specimens in the operating room.Methods:Established a HFMEA work team to identify topic,make work flow chart,find out the potential failure mode in routine pathological specimens management in the operating room,and analyze the reasons,develop action plan and implementation.Results:The quality indexes of specimen management(specimen confirmation specification compliance rate,accuracy rate of intra operative specimen temporary storage,accuracy of specimen registration,the qualified rate of inspection specimen)were significantly improved,and the differences were statistically significant(P<0.001).Conclusion:The application of HFMEA to the management of routine pathological specimens in the operating room can effectively guarantee the safety of pathological specimens and improve the quality of nursing in the operating room.
