温肾护元化瘀法联合护肾操治疗阳虚血瘀型糖尿病肾病的临床疗效观察

摘　　要：目的:探讨温肾护元化瘀法联合护肾操治疗阳虚血瘀型糖尿病肾病的临床疗效。方法:收集启东市中医院2017年12月-2020年2月收治的糖尿病肾病患者,根据中医辨证分型选取79例阳虚血瘀型患者作为研究对象,将其随机分为3组,A组给予基础对症治疗,B组在基础对症治疗的基础上给予中医药温肾护元化瘀法治疗,C组在基础对症治疗的基础上给予温肾护元化瘀法联合护肾操治疗。3个月后比较各组的空腹血糖、餐后2小时血糖、糖化血红蛋白、肾功能指标(尿素氮、肾小球滤过率)和中医症状积分。结果:B、C两组治疗后肾小球滤过率明显升高(P<0.05);B、C两组治疗后中医症状均有改善(P<0.01),且C组比B组改善更为显著(P<0.05)。结论:建立在西医基础上的中医药治疗在改善糖尿病肾病患者肾功能和中医症状方面发挥了一定的优势,值得临床推广运用。Objective:To investigate the clinical efficacy of the warming kidney and removing blood stasis method plus kidneyprotecting exercise on the Yang(阳)deficiency and blood stasis type diabetic nephropathy.Methods:79 cases from December 2017 to February 2020 were selected according to TCM syndrome differentiation,and were randomly divided into 3 groups.The group A was given basic symptomatic treatment,the group B was treated by the warming kidney and removing blood stasis method of TCM plus basic symptomatic treatment,and the group C was treated by the warming kidney and removing blood stasis method plus kidney-protecting exercise and basic symptomatic treatment.After 3 months,fasting plasma glucose,postprandial blood glucose,glycosylated hemoglobin,renal function indexes(urea nitrogen,glomerular filtration rate)and TCM symptom score were compared among the groups.Results:After treatment,the glomerular filtration rate was increased significantly in the group B and the group C(P<0.05).TCM symptoms in the group B and the group C were improved after treatment(P<0.01),and the improvement in the group C was more significant than that in the group B(P<0.05).Conclusion:Based on Western medicine,the TCM therapy shows a certain advantage in improving renal function and ameliorating TCM symptoms of patients with diabetic nephropathy,and is worthy of clinical promotion and application.
