橘核活精方治疗特发性弱精子症的临床研究

摘　　要：目的:观察橘核活精方治疗特发性弱精子症的临床疗效并进一步探索其作用机制和靶点。方法:将132例特发性弱精子症患者随机采用区组随机化方法分为两组。治疗组口服橘核活精方辨证论治,对照组则口服左卡尼汀口服液+多维元素片,4周为1个疗程,共治疗3个疗程。观察两组研究前、后精液常规[包括精子浓度、前向运动(Progressive,PR)精子百分率等]、生精细胞凋亡率、精子DNA碎片率等各参数。结果:临床研究前,两组间年龄构成、病程、精液常规、生精细胞凋亡率、精子DNA碎片率,经比较,其差异不具有统计学意义,两组组间基线特征均衡性良好,具有可比性。临床研究后,两组精液常规、生精细胞凋亡率、精子DNA碎片率指标均优于治疗前,差异具有统计学意义(P<0.05);治疗组各项指标改善程度较对照组显著(P<0.05)。结论:橘核活精方可以显著改善特发性弱精子症患者的精液常规、生精细胞凋亡率、精子DNA碎片率等参数,有着良好的临床疗效。Objective:To observe the clinical efficacy of Juhe Huojing formula(橘核活精方)in the treatment of idiopathic asthenospermia and to further explore its mechanism and target.Methods:A total of 132 patients with idiopathic asthenospermia were randomly divided into two groups by block randomization.The treatment group was treated by oral administration of Juhe Huojing formula according to syndrome differentiation,while the control group was treated with oral levocarnitine solution+multidimensional element tablets.The treatment lasted for 3 courses,which was of 4 weeks.The semen routine(including sperm concentration,PR sperm percentage,etc.),apoptosis rate of spermatogenic cells,sperm DNA fragmentation rate and other parameters of the two groups were observed before and after the study.Results:Before the clinical study,the differences in age composition,course of disease,semen routine,spermatogenic cell apoptosis rate and sperm DNA fragmentation rate between the two groups were not statistically significant,and the baseline characteristics of the two groups were well balanced and comparable.After the clinical study,the indexes of routine semen,apoptosis rate of spermatogenic cells and sperm DNA fragmentation rate in both groups were better than those before treatment,which was with statistical significance(P<0.05).The improvement of each index in the treatment group was significantly higher than that in the control group(P<0.05).Conclusion:Juhe Huojing formula can significantly improve the semen routine,spermatogenic cell apoptosis rate,sperm DNA fragmentation rate and other parameters of patients with idiopathic asthenospermia,and it has a good clinical effect.
