补虚活血通络方联合噻托溴铵治疗COPD迁延期临床疗效及其作用机制

摘　　要：目的:探讨补虚活血通络方联合噻托溴铵治疗慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)迁延期的临床疗效及对血清白三烯B4(leukotriene B4,LTB4)、白细胞介素8(interleukin-8,IL-8)水平的影响。方法:将85例COPD迁延期患者随机分为两组,因随访失败,最终联合组和单一组各40例,单一组行噻托溴铵粉吸入剂治疗,必要时予机械通气,联合组在单一组基础上予补虚活血通络方,观察治疗前后两组患者血清LTB4与IL-8水平、肺功能检测、嗜酸性粒细胞(eosinophilic granulocyte,EOS)计数及圣乔治呼吸问卷(St George’s Respiratory Questionnaire,SGRQ)与6 min步行距离实验(6minute walking distance,6MWD)问卷评分,并观察疗效。结果:两组治疗后血清LTB4与IL-8水平、SGRQ与6MWD问卷评分、用力肺活量(forced expiratory volume,FEV)、1秒用力呼气容积(forced expiratory volume in one second,FEV1)、呼气峰流量(peak expiratory flow,PEF)及临床疗效联合组优于单一组(P<0.05)。结论:补虚活血通络方联合噻托溴铵治疗COPD迁延期效果优于单纯噻托溴铵治疗,能减轻患者症状,改善相关指标。Objective:To investigate clinical effects of deficiency-tonifying blood-activating collateraldredging prescription and tiotropium bromide in the treatment of COPD at middle later stage and its influence on the levels of serum LTB4 and IL-8.Methods:All 85 patients were randmized into two groups,the combination group and the single group with 40 cases in each group consequently due to follow-up failure,the single group was treated by tiotropium bromide powder inhalant,mechanical ventiilation if necessary,the combination group took deficiency-tonifying blood-activating collateral-dredging prescription based on the therapy the single group accepted,to observe the levels of serum LTB4/IL-8,the detection of pulmonary function,EOS count and the scores of SGRQ,6MWD questionaire before and after treating,to assess clinical effects.Results:After treating,the levels of serum LTB-4/IL-8,the scores of SGRQ,6MWD questionaire,FEV,FEV1,PEF and clinical effects of the combination group were superior to these of the single group(P<0.05).Conclusion:Deficiency-tonifying blood-activating collateral-dredging prescription and tiotropium bromide in the treatment of COPD at middle later stage is superior to single Western medicine,it could relieve the symptoms and improve the related indexes.
